Sarepta's next chapter depends on siRNA data, not Elevidys

Sarepta is a near-breakeven Duchenne company whose gene therapy has shrunk and whose older drugs face an FDA ruling in February 2027. The investment case now rests on whether the Arrowhead siRNA platform works in DM1 and FSHD with repeat dosing, which the company says it will show in the second half of 2026.

Opinion only. The Investor Rank is aibio.space's opinion of the company's business position, drawn from the public sources on this page. It is not a recommendation to buy, sell or hold any security and it is not investment advice. It does not consider the share price, valuation or your circumstances, and it can change at any time. Payments from companies never affect it. How the rank works.

Technology
Proven in Duchenne, unproven beyond. Exon skippers and Elevidys are approved; the Arrowhead siRNA platform has only single-dose data showing dose-dependent muscle delivery.
Odds
Early stage for the growth story. The siRNA programs are in phase 1/2, where rare disease drugs reach approval about 17% of the time per BIO data.
Balance sheet
Adequate for now. About $945 million in cash against $848 million in convertible notes, with the company near breakeven on a non-GAAP basis.
Main risk
Two franchises under pressure. Elevidys has a boxed warning, and the exon-skipping drugs face an FDA ruling after a missed confirmatory endpoint.
Watch next
DM1 and FSHD repeat-dose data. Readouts are due in the second half of 2026, followed by the Feb. 28, 2027 FDA action date.

Notable gaps

Sarepta's upbeat second-quarter release is softer on ESSENCE and Elevidys risks than its 10-Q

The August release presents the exon-skipping drugs' regulatory filings as supported by the ESSENCE confirmatory study and calls the balance sheet strong. The 10-Q says ESSENCE missed its primary endpoint, warns that accelerated approvals could be revoked, and limits its funding statement to the next twelve months. Litigation is extensive and described in the 10-Q, including a securities class action.

  1. Press release vs filing · weight high

Release cites ESSENCE as support; the 10-Q says it missed its primary endpoint

The company said: "The applications are supported by data from the ESSENCE confirmatory study, substantial published real-world evidence, and the favorable, consistent safety profiles of both exon-skipping therapies." source

The record says: The 10-Q states: "In November 2025, we announced topline results from our ESSENCE trial, a confirmatory trial intended to verify the clinical benefits of two of our PMO Products: AMONDYS 45 and VYONDYS 53. The topline results did not show statistical significance on the study's primary endpoint." Its liquidity risk list names "revocation of accelerated approvals and directives to remove products from the market relating to the topline results of our ESSENCE trial that failed to meet statistical significance on its primary endpoint." source

Our view: The release's only mention of ESSENCE is as a source of support. It does not say the trial missed. The 10-Q is where an investor learns that the confirmatory study failed its primary endpoint and that withdrawal of the accelerated approvals is a disclosed risk. The FDA decision on the sNDAs is the event that settles this.

  2. Press release vs filing · weight medium

"Strong financial position" versus a twelve-month funding statement

The company said: "Strong financial position supports advancement of our pipeline and funding of medium-term liabilities" source

The record says: The 10-Q says cash, investments and operating cash flows are "sufficient to fund its current operational plan for at least the next twelve months," and that "Beyond June 30, 2027, our cash requirements will depend extensively on our ability to advance our research, development and commercialization of product candidates." It lists "the obligations to holders of our 2027 Notes and 2030 Notes" and the ability to "resume commercial shipments of ELEVIDYS for non-ambulatory patients" among the factors. The release reports GAAP operating income of $13.3 million against non-GAAP $86.5 million; the release defines non-GAAP as excluding depreciation, stock compensation and the litigation contingency charge, and the 10-Q records that charge at $39.0 million. source

Our view: $945.0 million of cash is real and the company had GAAP operating income of $13.3 million in the quarter, but a GAAP net loss of $4.9 million. Total revenue fell to $401.3 million from $611.1 million a year earlier, and the 10-Q does not extend its funding comfort past a year.

  3. Science · weight medium

Elevidys non-ambulatory restart depends on a single-arm cohort and FDA agreement

The company said: "For ELEVIDYS, full enrollment of ENDEAVOR Cohort 8 expected by year-end 2026 and 12-week data from the full cohort in Q1 2027." source

The record says: The 10-Q says: "Resumption of dosing in the non-ambulatory population will depend on the FDA's analysis of whether the sirolimus data positively changes ELEVIDYS' risk/benefit profile and on aligning with the FDA on the process for revising the label, both of which involve risks and uncertainties." ClinicalTrials.gov lists the ENDEAVOR study (NCT04626674) as an unmasked Phase 1 with an estimated 83 participants; its Cohort 8 primary outcome is "Number of Participants with Acute Liver Injury (ALI)" up to Week 72, plus dystrophin expression at Week 12. source

Our view: The release gives the data timing but not what the data must show to restart dosing. A small unmasked cohort with a liver injury count as a primary outcome can inform safety, but it will not show clinical benefit. The label now carries a boxed warning for acute liver injury.

  4. Science · weight high

ESSENCE was a large, masked trial on a functional endpoint

The record says: ClinicalTrials.gov lists ESSENCE (NCT02500381) as a Phase 3, quadruple-masked study with 228 participants enrolled and a single primary outcome of "Change From Baseline in the 4-Step Ascend Velocity at Week 96." Dystrophin protein levels by Western blot are listed as a secondary outcome. The 10-Q says the study missed statistical significance on its primary endpoint and that Sarepta submitted sNDAs for traditional approval in April 2026, accepted for filing in June 2026. source

Our view: This was the proper randomized test of whether the dystrophin-based accelerated approvals translate into function. The sNDAs lean on real-world evidence and safety instead. Whether FDA accepts that mix is the open question.

  5. Legal and disclosure record · weight high

Securities class action, derivative suits and patent cases are pending

The record says: Note 16 of the 10-Q describes a putative securities class action filed June 26, 2025 in the U.S. District Court for the Southern District of New York and transferred to the District of Massachusetts on November 6, 2025. The amended complaint covers purchasers between June 22, 2023 and November 3, 2025 and alleges statements about Elevidys safety, the LGMD programs and the ESSENCE trial were false or misleading. Sarepta moved to dismiss on March 9, 2026, and oral argument was held August 4, 2026. Derivative suits were filed in S.D.N.Y. (July 2025, stayed) and D. Mass. (April 7 and July 23, 2026). Patent suits by REGENXBIO and the University of Pennsylvania, Nippon Shinyaku and Genzyme are disclosed, with a Genzyme trial scheduled for June 14, 2027, and a $39.0 million charge for a potential patent settlement still subject to documentation. source

Our view: These are allegations in court filings that the company is contesting. No ruling on the motion to dismiss is disclosed. The ruling on the motion and the patent settlement paperwork are the next items to follow.
