Odds
Phase 3 base rates apply. TPIP is in phase 3, where about 52% of programs historically reach approval.

Near breakeven. About $1.2 billion in cash and securities, $543.5 million in debt, and a Q2 loss of only $13.2 million.

    Opinion · Devil's advocate

    Fact-check-o-meter

    Consistent

Insmed's messaging tracks its 10-Q, but the inhaled PAH drug story leans on uncontrolled data

The second-quarter release and the 10-Q agree on cash, revenue and the absence of any material litigation. The cautious points are on the science: the 12-month TPIP update comes from an open-label extension with no control arm, and the peak sales estimates in the release do not appear in the 10-Q.

    1. Science · weight medium

TPIP "safe and well-tolerated" headline sits on an uncontrolled extension study

The company said: "TPIP Was Safe and Well-tolerated with No Newly Identified Safety Signals through Month 12" source

The record says: The same release states "The OLE study is a non-placebo-controlled trial," and its safety paragraph adds: "Of the 91 patients in the study, treatment-emergent adverse events (TEAEs) occurred in 89.0% of patients; serious TEAEs were observed in 18.7% of patients." It also reports four deaths, "none of which were considered related to TPIP treatment," and discontinuations for TEAEs in 7.7% of patients. ClinicalTrials.gov lists the extension (NCT05649748) as unmasked with a safety primary outcome, so the walk-distance and NT-proBNP gains are not primary endpoints. source

Our view: Sustained gains in an open-label group of 91 patients are encouraging but cannot separate drug effect from patient selection or expectation. The adverse event rates are typical of a sick PAH population, and the release discloses them, but "safe and well-tolerated" is the company's reading. The placebo-controlled Phase 3 PALM-PAH is the real test.

    2. Science · weight medium

Phase 2b win was on a hemodynamic measure, not how patients function

The company said: "the statistically significant and clinically meaningful results from our Phase 2b randomized study" source

The record says: ClinicalTrials.gov lists the randomized, double-masked Phase 2 study (NCT05147805) with 102 participants and a single primary outcome: "Change from Baseline in Pulmonary Vascular Resistance at Week 16." The 10-Q says the Phase 3 PALM-PAH study began in April 2026 and is still enrolling. source

Our view: Pulmonary vascular resistance is an accepted early-phase measure in PAH, but it is a surrogate. Phase 3 will need to show benefit on outcomes such as walk distance or clinical worsening.

    3. Press release vs filing · weight medium

Peak sales targets appear in the release, not in the 10-Q

The company said: "We now expect that our three lead programs will generate more than $14 billion in combined peak sales." source

The record says: The 10-Q does not repeat any peak revenue estimate. It describes TPIP as in Phase 3 (PALM-ILD enrolling, PALM-PAH enrolling, a progressive pulmonary fibrosis study planned for the second half of 2026) and reports an accumulated deficit of $5.8 billion. The release's own forward-looking section lists "inaccuracies in our peak sales estimates for our marketed products and our product candidates" among its risks. source

Our view: Peak sales figures are company opinion, and the release says so in its caution language. A more-than-$6 billion TPIP figure applies to a drug with no Phase 3 data yet, so it reads as a company goal, not a forecast.

    4. Press release vs filing · weight low

Small quarterly loss versus a $176.8 million six-month loss

The company said: "For the second quarter of 2026, Insmed reported a net loss of $13.2 million, or $0.06 per share, compared to a net loss of $321.7 million, or $1.70 per share, for the second quarter of 2025." source

The record says: The 10-Q confirms the $13,242 thousand quarterly loss and reports a six-month net loss of $176,805 thousand. Management states: "we anticipate that our cash and cash equivalents and marketable securities as of June 30, 2026 will enable us to fund our operations for at least the next 12 months." The filing carries no going-concern doubt language. source

Our view: The release numbers are accurate and the quarter was near break-even. The first quarter was much weaker, and the funding statement in the 10-Q is a 12-month one. One quarter is a data point rather than a trend.

    5. Legal and disclosure record · weight low

      No material litigation disclosed

The record says: The 10-Q Legal Proceedings note states: "From time to time, the Company is a party to various lawsuits, claims and other legal proceedings that arise in the ordinary course of business. While the outcomes of these matters are uncertain, management does not expect that the ultimate costs to resolve these matters will have a material adverse effect." No securities litigation or enforcement disclosed in the latest filing. source

Our view: The record is clean in the filing we read. Arikayce's US label is still an accelerated approval, which the 10-Q notes, so the sNDA submitted in July is a regulatory item to follow.
