Gilead bought its next act. Now it has to sell it
Gilead has a growing HIV base and a run of 2026 approvals, but it spent most of its cash on deals that still have to prove themselves. The anito-cel decision in December and Yeztugo persistence are the two numbers that will say whether the bet pays.

It is not a recommendation to buy, sell or hold any security and it is not investment advice. It does not consider the share price, valuation or your circumstances, and it can change at any time. - Technology
- Long-acting HIV leader. Lenacapavir now underpins a twice-yearly PrEP shot, a daily tablet and a weekly oral regimen that matched Biktarvy in phase 3.
- Odds
- Mostly late stage. Anito-cel is filed with a December 23 PDUFA date, and filed drugs historically reach approval about 90% of the time.
- Balance sheet
- Profitable but stretched. Cash fell to $3.18 billion at June 30 from $10.6 billion after about $11 billion in deals, with new debt taken on.
- Main risk
- HIV concentration. About three quarters of product sales come from HIV, and new products partly move revenue from Biktarvy.
- Watch next
- Q3 then anito-cel. Results land October 29 and the anito-cel FDA decision is due December 23.

Fact-check-o-meter
Gilead's releases mostly match its 10-Q, with thin topline data and an impairment the release does not tie to the lung study
Gilead's second-quarter release gave GAAP and non-GAAP figures side by side and disclosed the EVOKE-03 lung cancer stop, so the big numbers line up with the 10-Q. The softer spots are science ones: the weekly HIV pill was announced with no efficacy figures, and the new first-line Trodelvy label rests on progression-free survival while overall survival is still being followed.

Upbeat quarter quote sits beside a $1.75 billion write-off
The company said: "We also made significant clinical progress with three FDA approvals and three positive Phase 3 updates." source
The record says: The 10-Q ties the charge directly to the failed lung cancer study: "In consideration of this decision, and in connection with the preparation of the financial statements for the second quarter of 2026, we determined that no future cash flows were expected to be generated in relation to the NSCLC IPR&D intangible asset, and therefore, no value was attributed to the asset. As a result, we recognized an impairment charge of the remaining balance of $1.75 billion." The release lists the EVOKE-03 discontinuation in its pipeline section but describes the charge in its narrative text only as "an IPR&D impairment related to assets previously acquired from Immunomedics, Inc." source
Our view: Nothing is hidden here, since both facts are in the release. But a reader has to connect them alone, and the 10-Q is the place that says the first-line lung cancer value of the Immunomedics deal is now written to zero.

"Very strong" quarter was a $10.5 billion GAAP loss
The company said: "Gilead delivered a very strong second quarter, with 10% year-over-year revenue growth in our base business driven by our HIV portfolio, Trodelvy and Livdelzi." source
The record says: The 10-Q income statement shows a net loss of $10,496 million for the three months ended June 30, 2026, against net income of $1,960 million a year earlier, driven by acquired IPR&D from the Arcellx, Tubulis and Ouro Medicines deals. The 10-Q also notes Biktarvy growth was "primarily due to higher average realized price, favorable inventory dynamics and higher demand." source
Our view: The release does print the $(8.45) GAAP loss per share in its sub-headline, so this is framing, not omission. The operating business did grow; the loss is deal accounting. The inventory and price contribution to Biktarvy is worth tracking because the 10-Q warns second-half wholesaler buying is often followed by draw-downs.

Weekly HIV pill topline came with no numbers
The company said: "In the double-blind ISLEND-1 trial, the once-weekly, single-tablet regimen of ISL/LEN was found to be statistically non-inferior to BIKTARVY. In the open-label ISLEND-2 trial, ISL/LEN was found to be statistically non-inferior to standard of care daily oral antiretroviral therapy regimens." source
The record says: ClinicalTrials.gov lists ISLEND-2 (NCT06630299) with no masking (open label) and an estimated enrollment of 600; ISLEND-1 (NCT06630286) enrolled 609 and is double-blind. Both records list change from baseline in CD4 T-cell count at Week 48 as a secondary outcome. The June 8 release gives no virologic failure rates, no CD4 data and no discontinuation rates, only that safety was "generally comparable" and that no new safety concerns were identified. source
Our view: Non-inferiority in suppressed switch patients is the expected bar, and one of the two trials is blinded, which helps. Until the snapshot rates and CD4 changes are presented at a congress, investors are taking the safety summary on the companies' word.
Science · weight medium
First-line Trodelvy approval rests on PFS, survival still open
The company said: "The FDA approval is based on highly statistically significant and clinically meaningful progression-free survival (PFS) data from the Phase 3 ASCENT-03 and ASCENT-04/KEYNOTE-D19 trials" source
The record says: ClinicalTrials.gov lists ASCENT-03 (NCT05382299) as open-label (no masking), with PFS by blinded central review as the primary outcome, "Overall Survival (OS)" as a secondary outcome, and an estimated primary completion of July 2028. The release cites overall survival benefits only for second-line TNBC and pre-treated HR+/HER2- disease. Its safety section reports fatal adverse reactions in 2.5% of ASCENT-03 patients and 3.2% of ASCENT-04 patients. source
Our view: PFS read by blinded central review is a standard basis for approval in this setting, and the release is clear that PFS is what was shown. The first-line survival result is the open question for how durable this label and the NCCN preference will be.
Legal and disclosure record · weight low
Antitrust, product liability and qui tam cases, no securities suit
The record says: Note 10 of the 10-Q discloses consolidated HIV antitrust class actions in the U.S. District Court for the Northern District of California (Phase I verdict for Gilead, appeal argued October 2025, Phase II stayed); an Aetna opt-out suit in San Mateo County Superior Court with trial "scheduled for January 2027"; product liability suits over tenofovir drugs that "involve approximately 23,000 active plaintiffs," with a federal bellwether trial set for March 2027; and a Texas Medicare Fraud Prevention Act qui tam suit in which "the Texas Attorney General has intervened as a plaintiff. Trial has been scheduled for December 2026." It also discloses a February 2026 patent suit against Cipla in the District of Delaware over Descovy. No securities class action or SEC enforcement matter is disclosed. source
Our view: These are long-running matters that Gilead discloses in detail, and the 10-Q says it had no material accruals for them at June 30. The dated items to watch are the Texas trial in December 2026 and the Aetna and federal bellwether trials in early 2027.
