BioMarin bets on breadth while Voxzogo faces its first rival
BioMarin is a profitable rare disease company that used debt to pay for growth through Amicus, and Voxzogo is near $1 billion a year with a second indication filed. The weak spot is the pipeline, where its most recent late-stage bet failed and the next readouts sit in 2027 and 2028.

Fact-check-o-meter
BioMarin's messaging mostly matches its filings, but the guidance raise and the Voxzogo pipeline story have moved since August
The August release disclosed that the revenue jump came from the Amicus acquisition, and the 10-Q backs the numbers. The gaps are about timing and framing: raised non-GAAP EPS guidance that excluded the later Alesta charge, a Voxzogo growth story that has already dropped development in Noonan syndrome, and a hypochondroplasia filing built on a small trial. The only legal item is a DOJ subpoena, with no securities litigation disclosed.

Raised EPS guidance predates a large new charge
The company said: "Stronger Growth Expectations Drive Increased Guidance for Full-year 2026 Total Revenues, VOXZOGO ® , and Non-GAAP Diluted Earnings Per Share (EPS)" source
The record says: The October 8-K says BioMarin expects "approximately $283 million of acquired in-process research and development (IPR&D) expense", "an unfavorable impact of approximately $1.50 to both GAAP Diluted earnings per share (EPS) and Non-GAAP Diluted EPS for the third quarter of 2026", and that the August 6 guidance "did not reflect the acquisition of Alesta". source
Our view: The August release raised non-GAAP EPS guidance. About two months later the 8-K said the Alesta acquisition, announced after the August release and completed August 31, would have an unfavorable impact of about $1.55 a share on full-year 2026 GAAP and non-GAAP EPS, which the August guidance did not reflect. The company said it expects to update guidance with third-quarter results.

Revenue growth is mostly bought, not organic
The company said: "Second Quarter 2026 Total Revenues Increased 20% Year-over-year to $990 million" source
The record says: The same release says the increase was "primarily driven by revenues from GALAFOLD and POMBILITI + OPFOLDA, which were acquired from Amicus on April 27, 2026", that VOXZOGO sales were $253 million against $221 million (up 14%), and that "GAAP Net Income for the second quarter of 2026 decreased to $45 million compared to $241 million for the same period in 2025." source
Our view: The release is open about the source of growth, so this is a framing point, not a contradiction. A 20% headline sits next to a 14% Voxzogo increase and a large fall in GAAP profit after acquisition and interest costs.
Press release vs filing · weight medium
Voxzogo's second indication, and the one dropped
The company said: "Adding to this momentum is the opportunity to advance our second potential indication with VOXZOGO, hypochondroplasia, based on recent pivotal data that exceeded our expectations." source
The record says: The 10-Q filed the same day lists hypochondroplasia, idiopathic short stature and Noonan syndrome as areas BioMarin is "continuing to advance". On September 15, 2026 an 8-K reported "its decision to discontinue development of VOXZOGO for children with Noonan syndrome" and said "This decision was not related to the safety or efficacy of VOXZOGO." source
Our view: The Voxzogo expansion story is now narrower than the August documents described, and the 8-K gives the reasons as the feasibility of the study and the current treatment landscape, and says the decision was not related to safety or efficacy. The 10-Q risk factor ties Voxzogo to larger patient populations, and one of the three conditions it names is now dropped.
Science · weight medium
Hypochondroplasia rests on a small, 52-week growth trial
The company said: "the Phase 3 CANOPY-HCH-3 study of VOXZOGO in children with hypochondroplasia met its primary endpoint, with a statistically significant increase in annualized growth velocity (AGV) at week 52 versus placebo (LS mean difference +2.33 cm/yr, p<0.0001)" source
The record says: ClinicalTrials.gov lists the trial (NCT06455059) with an estimated enrollment of 80, a primary outcome of "Change from baseline in annualized growth velocity (AGV) at Week 52 versus placebo", and a quadruple-masked randomized design. The release also cites a Phase 2 investigator-sponsored extension in 13 children. source
Our view: The result is placebo-controlled and statistically clear. It is still a one-year growth velocity measure in about 80 children, and the supporting long-term data come from 13. Final adult height is not what was tested.
Science · weight medium
U.S. Voxzogo approval is accelerated and the fix is open-label
The company said: "In July, BioMarin announced that the FDA accepted its sNDA for full approval of VOXZOGO in children with achondroplasia, with a Prescription Drug User Fee Act (PDUFA) target action date of February 28, 2027." source
The record says: The 10-Q says "GALAFOLD and VOXZOGO are approved in the U.S. under accelerated approval and are subject to post-marketing requirements to obtain full approval" and "To fulfill this post-marketing requirement, we intend to use our ongoing open-label extension studies compared to available natural history." source
Our view: The release frames February 2027 as a date for full approval. The filing adds that continued approval is contingent on confirmatory evidence, and that the evidence plan is open-label extensions against natural history. Voxzogo is roughly a quarter of quarterly revenue, so this date matters.
Legal and disclosure record · weight low
DOJ subpoena on testing programs, no securities litigation
The record says: The 10-Q reports that BioMarin "received a subpoena from the U.S. Department of Justice (DOJ) requesting that the Company produce certain documents regarding sponsored testing programs relating to VIMIZIM and NAGLAZYME", first disclosed in the 2023 10-K, that it is cooperating, and that it cannot give assurances about the outcome. Item 1, Legal Proceedings, reads "None." No securities litigation is disclosed in the latest filing. source
Our view: The subpoena is a documented, unresolved inquiry, now more than two years old. It concerns two older enzyme products, not Voxzogo, and no charge or finding is disclosed.
