United Therapeutics is buying back stock while it waits on two FDA filings
United Therapeutics has a shrinking-to-flat inhaled prostacyclin business, two filed applications backed by strong phase 3 data, and $3.80 billion of cash that it is steadily returning to shareholders. The Liquidia settlement removes a major legal threat and brings in $330.4 million. Whether the franchise decline outruns the 2027 approvals the CEO expects is the open question.

Fact-check-o-meter
United Therapeutics' filings back its trial claims, but the upbeat release language sits next to falling Tyvaso sales
The trial figures in the 10-Q match the company's public statements, and both big phase 3 programs were randomized and double-blind per ClinicalTrials.gov. The gaps are tonal: the Q2 release opens with the 2% revenue decline, and the executive quotes that follow stress the two NDA filings, multi-billion-dollar catalysts and record Tyvaso DPI demand, while the filing says competitive therapies negatively impacted sales. The Liquidia settlement filed in October is the biggest legal change since the 10-Q.

'Record' Tyvaso DPI demand, but Tyvaso and total revenue fell
The company said: "Tyvaso DPI exited the second quarter at record levels of starts, referrals, commercial patients, and total patients, reflecting strong underlying demand," source
The record says: The 10-Q attributes a Nebulized Tyvaso drop to a "decrease in U.S. quantities sold of $37.6 million" and adds: "We believe the availability of competitive therapies negatively impacted sales of Nebulized Tyvaso, Tyvaso DPI, and Remodulin for the three and six months ended June 30, 2026." source
Our view: The release itself reports the 2% revenue decline and repeats the competition language, so nothing is hidden (total Tyvaso revenue fell 4% to $452.6 million, per the release). But the release opens with the revenue decline, and the two executive quotes that follow stress the NDA filings and record Tyvaso DPI starts, while U.S. quantities sold fell for Nebulized Tyvaso and Remodulin.

IPF results are strong on lung function, but survival did not reach significance
The company said: "two potentially transformative, multi-billion-dollar catalysts that could significantly enhance our growth profile." source
The record says: The 10-Q reports FVC gains of 130.1 mL in TETON-1 and 95.6 mL in TETON-2, then states: "Overall survival at week 52 trended in favor of Nebulized Tyvaso but did not meet statistical significance." ClinicalTrials.gov lists TETON-1 as masked ("QUADRUPLE") with 598 participants and a single primary outcome, "Change in Absolute FVC from Baseline to Week 52". source
Our view: The design is sound: randomized, double-blind and placebo-controlled with a clear lung-function endpoint. The two trials differed by about 35 mL, and FVC is a surrogate for how patients live and survive. Regulators may weigh the survival trend, and the 'multi-billion' figure is the company's own forecast.

Ralinepag's win rests on a composite clinical worsening endpoint
The company said: "The study met its primary endpoint, with ralinepag reducing the risk of a clinical worsening event by 55 percent compared with placebo in patients with PAH (hazard ratio 0.45, p<0.0001)." source
The record says: The ClinicalTrials.gov primary outcome is "Time from randomization to the first adjudicated protocol-defined clinical worsening event", defined as "death, nonelective hospital admission for worsening PAH", prostacyclin initiation, "disease progression", or "unsatisfactory long-term clinical response". The trial is randomized and quadruple-masked with 687 participants. source
Our view: Composite endpoints can be driven by the softer components, and neither source here splits the 55% reduction by component. A breakdown by component and the 6-minute walk result would show how much the composite rests on its softer components.
Legal and disclosure record · weight medium
Liquidia and Sandoz matters settled in October; releases take effect after payments, a label change and dismissals
The record says: The October 14 8-K says "On September 30, 2026, the District Court held Liquidia liable for infringement of claims 1 and 14 of the '327 patent" in the U.S. District Court for the District of Delaware. Under the October 13 settlement, "Liquidia will pay to UT an aggregate of $330.4 million" and UT will make a "$10.0 million payment" to Liquidia to settle the RareGen and Sandoz matter, after the Third Circuit on October 7 "vacating the November 1, 2024 judgment and damages award". No securities litigation or enforcement action is disclosed in the 10-Q; its legal note lists a payor class action under RICO and antitrust law in the Southern District of Florida. source
Our view: The August results release did not mention Liquidia, and the 10-Q still described the cases as pending. The settlement removes the largest dispute and adds cash, but it depends on Liquidia making payments and on FDA approving a label change.
