Arrowhead's plozasiran moves from rare disease toward a mass market

Arrowhead has a strong late-stage drug, a filing coming before year-end and more than $1.5 billion in cash resources. It also has convertible debt, rising costs and tiny product sales so far. The next test is the FDA filing and what the label says about blood sugar.

Opinion only. The Investor Rank is aibio.space's opinion of the company's business position, drawn from the public sources on this page. It is not a recommendation to buy, sell or hold any security and it is not investment advice. It does not consider the share price, valuation or your circumstances, and it can change at any time. Payments from companies never affect it. How the rank works.

Technology
Proven in the liver. RNAi plozasiran is approved in five markets for FCS and cut triglycerides 79% to 81% in sHTG.
Odds
Mostly cleared. The trials are won and the filed-application approval rate is 90.6%, but earlier pipeline assets face phase I to approval odds of 7.9% to 17.0%.
Balance sheet
Large but leveraged. Cash resources were $1.57 billion, against $682.7 million of convertible notes and a $215 million voucher purchase.
Main risk
Commercial scale-up. Redemplo made about $2.4 million in the June quarter, per MarketBeat's summary of the earnings call, and the sHTG launch will need far more sales spending.
Watch next
sNDA filing before end of 2026, and closing of the voucher purchase.

Some gaps

Arrowhead's 100% pancreatitis headline rests on a subgroup of a secondary endpoint

Arrowhead's quarterly release and 10-Q agree on the numbers, and the 10-Q reports sufficient liquidity for twelve months with no going concern language. The gaps are in how the sHTG phase 3 data are framed: the pancreatitis result is a secondary endpoint, the 100% figure comes from a subgroup, and the dedicated pancreatitis trial has not read out. The only legal matter disclosed is a patent complaint from Ionis.

  1. Science · weight high

Pancreatitis is a secondary endpoint, and the 100% figure is a subgroup

The company said: "Statistically significant 78% reduction in acute pancreatitis (AP) events versus placebo was observed across the entire sHTG study population, with an unprecedented 100% event reduction in patients with triglycerides above 880 mg/dL and a prior medical history of AP, widely considered to be at the highest risk for AP." source

The record says: ClinicalTrials.gov lists "Adjudicated AP Event Rate During the Treatment Period Compared to Placebo From Day 1 to Month 12" as a secondary outcome in SHASTA-3 (446 participants) and SHASTA-4 (311), with triglyceride change as the primary outcome. A separate trial, SHASTA-5, lists time to first adjudicated AP event as its primary outcome; it is recruiting 288 participants (estimated) with primary completion estimated for March 2029. The Aug. 30 ESC release reports the pooled all-events result as a 78% reduction (RR 0.22, 95% CI 0.07 to 0.67) with an absolute risk reduction of 4.1%. source

Our view: A pooled secondary endpoint is useful evidence, and the confidence interval is wide. A subgroup showing zero events says little about the size of the effect, and SHASTA-5 is the study built to test the pancreatitis claim directly.

  2. Press release vs filing · weight medium

"All prespecified secondary endpoints" is not defined in the release

The company said: "SHASTA-3 and SHASTA-4 successfully met the primary endpoint of triglyceride reduction versus placebo and met all prespecified secondary endpoints in both studies." source

The record says: ClinicalTrials.gov lists 20 secondary outcomes for SHASTA-3, including new-onset diabetes, worsening of existing diabetes, HbA1c, fasting glucose, MACE rates and anti-drug antibodies, alongside the lipid and pancreatitis measures. The 10-Q repeats the same bullet and does not list which secondary endpoints were tested. source

Our view: The statement likely refers to the tested secondary hierarchy, but the registry list is much longer. Until the full results are posted, readers cannot tell which measures the sentence covers.

  3. Science · weight medium

"No new safety signals" sits beside a glycemic imbalance and three deaths

The company said: "Continued and consistent safety and tolerability profile was demonstrated with no new safety signals and a favorable liver safety profile." source

The record says: The Aug. 30 ESC release reports worsening glycemic control as an adverse event in 14.3% of plozasiran patients versus 8.7% on placebo in the pooled analysis, and three deaths in the plozasiran group (two cardiovascular, one from chronic myelomonocytic leukemia) that the release says were assessed as unrelated to study treatment. ClinicalTrials.gov lists new-onset diabetes, worsening diabetes and HbA1c as secondary outcomes. source

Our view: The ESC release discloses these figures, so this is about emphasis rather than omission. Glycemic effects matter for a drug aimed at a population with high diabetes rates, and they may figure in the label discussion.

  4. Press release vs filing · weight low

Redemplo momentum is described in prescriptions, while revenue is mostly partner payments

The company said: "REDEMPLO prescription volume roughly doubled during the fiscal third quarter." source

The record says: The 10-Q says the revenue change "was primarily driven by revenue recognition associated with the Sarepta, Novartis, Madrigal and Sanofi collaboration and license agreements, and partially driven by commercial revenue from REDEMPLO." Collaboration revenue was $72.9 million of $75.3 million total revenue for the quarter. Net loss attributable to Arrowhead was $194.3 million, and the 10-Q says liquidity is expected to be "sufficient" for "at least the next twelve months". source

Our view: Doubling from a small base is accurate but gives no dollar scale. Neither the release nor the 10-Q states a dollar figure for Redemplo sales. Total revenue of $75.253 million less collaboration revenue of $72.868 million leaves about $2.4 million, which the 10-Q does not label as Redemplo sales.

  5. Legal and disclosure record · weight medium

Ionis patent complaint over plozasiran; no securities litigation disclosed

The record says: Note 7 of the 10-Q says: "On September 11, 2025, Ionis filed a Complaint for Patent Infringement against the Company in the United States District Court for the Central District of California alleging patent infringement of the '333 patent by the Company's planned commercialization of investigational plozasiran and seeking damages." The company "disputes the allegations of wrongdoing" and no contingent liabilities were recorded as of June 30, 2026. No securities litigation or enforcement is disclosed in the latest filing. source

Our view: The Ionis case targets the same drug at the center of the sHTG filing plan. The 10-Q gives no timetable or damages estimate, so any ruling would be a new fact for investors.
