Fractyl's Revita has its trial win and a funding gap
Revita cleared its first registrational test, but the benefit over sham is a few points of body weight and the company's cash runs out before FDA is due to decide. Fractyl is now a one-product story whose next year turns on the 12-month data and on how it funds the gap to a possible 2028 launch.

- Technology
- Positive but modest. Revita patients regained 5.1% of body weight against 8.2% on sham at six months in a 308-patient registrational trial.
- Odds
- Reasonable on the filing. One co-primary endpoint is met, and the one-year midpoint cohort showed a 73% responder rate against a 50% FDA goal.
- Balance sheet
- Short. Cash funds operations into the second quarter of 2027, before an FDA decision expected in late 2027.
- Main risk
- Financing before revenue. Fractyl will likely need new capital or a partner to reach a launch planned for early 2028.
- Watch next
- De Novo filing in late Q4 2026. Then 12-month registrational data in the first quarter of 2027.

Fact-check-o-meter
Fractyl's win is real but narrow, and the release leans on post hoc cuts while going-concern doubt sits in the 10-Q
REMAIN-1 met its first co-primary endpoint against sham, which is a genuine result. The release also puts post hoc subgroup figures in its headline bullets, and the second co-primary endpoint, which the FDA-mandated goal depends on, is not due until Q1 2027. The latest 10-Q states substantial doubt about going concern, which the October releases do not repeat.
Science · weight high
Headline benefit rests partly on post hoc cuts
The company said: "Post hoc analyses demonstrated that patients receiving a single complete duodenal ablation with Revita (>=20 cm length) regained 3.8% total body weight vs 8.2% in sham, reflecting a 53% reduction in weight regain between Month 1 and Month 6 (nominal p=0.003)" source
The record says: The same release gives the prespecified result: "least-squares mean weight regain of 5.1% versus 8.2% of body weight; n=176 versus 74" (p=0.0023). It also states that "key secondary endpoints are formally evaluated for statistical inference following assessment of the 12-month co-primary endpoint." source
Our view: The prespecified gap is about 3 percentage points of body weight, not the 4.4 points in the ablation-length cut that picks patients by what the procedure achieved. Those subgroups have not yet been formally tested, so the larger numbers should be read as hypotheses.
Science · weight high
Second co-primary endpoint is still open
The company said: "REMAIN-1 met its six-month co-primary endpoint with high statistical significance (p=0.0023; mITT population)" source
The record says: The release describes the second co-primary endpoint as "a responder rate defined as the proportion of Revita participants maintaining at least 5% total body weight loss relative to their pre-tirzepatide weight at one year, assessed against an FDA-mandated pre-specified performance goal of greater than 50%." The registry record for the trial lists a 52-week responder outcome alongside the week 26 outcome. source
Our view: The study has two co-primary endpoints, and the second one is assessed against a fixed performance goal, not against sham. A De Novo filing is planned for late Q4 2026, before that result exists.
Science · weight medium
Population and durability limits
The company said: "Revita has been generally well-tolerated with no device- or procedure-related serious adverse events" source
The record says: The release says the study enrolled "adults with obesity (BMI 30 to 45) without type 2 diabetes who had not previously received GLP-1 therapy", all given tirzepatide first, and that safety is "as of the September 24, 2026, data cut-off date" in an ongoing study. The primary result is at six months. source
Our view: The sham-controlled design is a strength. The safety statement is interim, and the six-month effect in a tirzepatide-primed, diabetes-free group may not carry over to longer follow-up or to patients with diabetes.
Press release vs filing · weight high
Going-concern doubt and the runway language
The company said: "Cash runway extended into Q2 2027; quarterly operating cash burn expected to be reduced by ~50% to approximately $8 million by end of Q2 2027" source
The record says: The 10-Q says cash of $47.1 million at June 30, 2026 would fund operations "into early 2027" and "substantial doubt exists about the Company's ability to continue as a going concern for at least one year after the date that these financial statements are issued." The 8-K adds about $2 million of severance charges. source
Our view: Q2 2027 is still before the planned late 2027 authorization and early 2028 launch, and the October release does not say whether the going concern language in the 10-Q would change. It did not mention going concern.
Press release vs filing · weight medium
Rejuva milestone dropped within weeks
The company said: "Company discontinuing development of Rejuva candidates and seeking strategic alternatives to maximize the value of its Rejuva gene therapy program" source
The record says: The 10-Q said the company was "well funded through multiple key clinical and regulatory milestones in 2026, including ... initial dosing and preliminary data from our RJVA-001 clinical program." source
Our view: The August filing counted RJVA-001 data as a 2026 milestone that the funding would cover. On October 22 the company announced it was discontinuing internal development of Rejuva, in a release that also announces a 45% workforce reduction and lower cash burn.
Legal and disclosure record · weight medium
Nasdaq delisting notice, no litigation
The record says: The 10-Q states "We are not subject to any material legal proceedings." An 8-K dated September 10, 2026 says Nasdaq notified the company by letter that its securities "are subject to delisting from Nasdaq unless the Company timely requests a hearing" because the bid price stayed under $1.00; the company said it intends to request a hearing. Later 8-Ks report that stockholders approved a reverse split range on September 24, 2026 and that a one-for-twelve reverse split took effect on October 2, 2026. No securities litigation or enforcement is disclosed in the latest filing. source
Our view: The notice concerns the Nasdaq minimum bid price rule and is not a legal claim. Neither October release mentions the delisting process.
