Lilly's growth is real; price, debt and retatrutide's label are the tests
Lilly is the volume leader in obesity drugs and its next products, Foundayo and retatrutide, are approved or late-stage. The risks are falling net prices, rising debt and the safety profile retatrutide will carry onto its label.

Opinion only. The Investor Rank is aibio.space's opinion of the company's business position, drawn from the public sources on this page. It is not a recommendation to buy, sell or hold any security and it is not investment advice. It does not consider the share price, valuation or your circumstances, and it can change at any time. Payments from companies never affect it. How the rank works.
- Technology
- Proven at scale. Tirzepatide drives about 65% of Q2 revenue, and the oral Foundayo pill is approved for obesity.
- Odds
- High for the late-stage book. Retatrutide has a complete phase 3 package; filings to approval succeed about 90.6% of the time in industry data.
- Balance sheet
- Profitable but more leveraged. Cash of $8.95 billion at June 30 against about $55 billion of total debt after a deal-heavy year.
- Main risk
- Net price erosion. Realized prices fell 13% in Q2 as Lilly widened access and competition with Novo Nordisk intensified.
- Watch next
- Q3 results on October 29. Guidance, Foundayo uptake and any deal charges are the numbers to watch.

Michigan Supreme Court reverses ruling for Lilly in insulin investigation case
The 10-Q filed 2026-08-05 reports that in July 2026 the Michigan Supreme Court reversed a Michigan Court of Appeals decision that had affirmed a judgment in Lilly's favor. The case concerns whether the Michigan attorney general has authority to investigate Lilly's insulin sales under the Michigan Consumer Protection Act. Separately, the 10-Q says most insulin pricing cases, including putative class actions, have been coordinated or consolidated in a multidistrict litigation in the District of New Jersey.

Supreme Court declines review of average manufacturer price verdict against Lilly
In November 2014 a relator filed a qui tam action in the Northern District of Illinois against Lilly and Takeda Pharmaceuticals America. The relator's complaint alleged that the defendants should have treated certain distributor credits as retroactive price increases in calculating average manufacturer prices. A jury returned a verdict for the relator in August 2022. The Seventh Circuit affirmed in September 2025, and Lilly recognized a charge. The 10-Q filed 2026-08-05 says the U.S. Supreme Court denied further review in May 2026.

The 10-Q filed 2026-08-05 says the matter is ongoing. It also describes other 340B lawsuits against Lilly and a pending Seventh Circuit appeal.

Fact-check-o-meter
Lilly's releases and its 10-Q tell the same story, and the releases carry most of their own caveats
The Q2 release, the retatrutide release and the Foundayo comparison release match the 10-Q and the trial records where we could check them. The price drop, the acquired R&D charge and the limits of an indirect comparison are all stated in the releases themselves. What a reader of the releases alone would miss is the growth in borrowing and the product liability suits over tirzepatide, which sit in the 10-Q.
Press release vs filing · weight low
Price erosion is disclosed in both documents
The company said: "Excluding these adjustments, U.S. price would have declined by approximately 9%." source
The record says: The 10-Q says U.S. "lower realized prices were primarily driven by Zepbound and Mounjaro, partially offset by adjustments to estimates for rebates and discounts primarily driven by Trulicity, Mounjaro, and Zepbound". source
Our view: The release leads with 48% revenue growth but states the 13% fall in realized prices and the 9% underlying U.S. decline in the same section. The filing agrees. This is consistent messaging, and the open question is how long volume can outrun price.
Press release vs filing · weight low
Borrowing growth is in the filing, not the release
The company said: "Increased 2026 full-year revenue guidance to be in the range of $85.0 billion to $87.0 billion" source
The record says: The 10-Q balance sheet shows long-term debt of $47,858 million at June 30, 2026 against $40,868 million at December 31, 2025, and short-term borrowings and current maturities of $7,050 million against $1,635 million. The release says the $3.03 per share of acquired IPR&D charges more than offset the raised underlying non-GAAP EPS guidance. source
Our view: The release is open about the acquired R&D charge but does not discuss the debt that funded the deals. Investors who read only the release would not see the balance sheet change.

Foundayo versus oral semaglutide is an indirect comparison
The company said: "Lilly's Foundayo (orforglipron) 17.2 mg showed greater weight loss and A1C reduction than oral semaglutide 25 mg among adults with type 2 diabetes in a new indirect treatment comparison" source
The record says: ClinicalTrials.gov lists ACHIEVE-3 (NCT06045221) with masking "NONE", and the release itself calls it "open-label". The release says "A 25 mg once-daily dose of oral semaglutide is not approved in the United States for the treatment of type 2 diabetes" and that data use the efficacy estimand. The weight-loss gain was 1.5% with a 95% CI of -2.7% to -0.2%. The registry lists orforglipron doses of 12 mg and 36 mg, while the release uses 9 mg and 17.2 mg, and the release does not explain the labels. source
Our view: The headline is accurate, and the release's own quote from a Lilly executive says Foundayo "may deliver A1C and weight lowering that is similar to the 25 mg dose". An adjusted cross-trial comparison with a wide interval is weaker evidence than the headline suggests.
Science · weight medium
Retatrutide: weight loss met, cardiovascular benefit not shown
The company said: "major adverse cardiovascular events (MACE) occurred less frequently than anticipated in both retatrutide and placebo arms" source
The record says: The release reports "Discontinuation rates due to adverse events in TRIUMPH-3 were 9.8% (9 mg) and 13.5% (12 mg)." Placebo was 4.8%. The release also reports a MACE-3 hazard ratio of 1.12 (95.0% CI: 0.64 to 1.96) for pooled retatrutide against placebo. ClinicalTrials.gov lists the TRIUMPH-3 primary outcome (NCT05882045) as percent change in body weight at week 80, double-masked, with 1,946 actual participants. source
Our view: The release does not claim a cardiovascular benefit and gives the discontinuation rates, so there is no gap with the record. The trial was sized and designed for weight loss, and the higher dropout on drug is the number to watch when the BLA is filed.
Legal and disclosure record · weight medium
Tirzepatide product liability MDLs and generic filings disclosed
The record says: Note 9 of the 10-Q: "Most of these lawsuits in the United States have been coordinated or consolidated for pretrial proceedings in two federal MDLs: one focused on alleged gastrointestinal injuries, and the other relating to claims of non-arteritic anterior ischemic optic neuropathy (NAION). Both MDLs are pending in the U.S. District Court for the Eastern District of Pennsylvania." It also says "In July 2026, Lilly received notice that multiple generic companies had filed ANDAs seeking approval to market generic versions of Mounjaro and/or Zepbound". source
Our view: These are matters Lilly discloses itself. The litigation note does not describe a securities class action, and the Q2 release does not mention the product liability MDLs or the generic filings.
