Praxis faces two FDA decisions five weeks apart

Praxis says the FDA found no major concerns at mid-cycle for either application, and it expects cash to last into 2028. The vormatrigine miss in June and the three-month relutrigine extension are the setbacks so far.

The technology

Praxis builds neurology drugs on two platforms: Cerebrum small molecules and Solidus antisense oligonucleotides. Ulixacaltamide is a selective inhibitor of T-type calcium channels, designed to block abnormal neuronal burst firing in a brain circuit tied to tremor. Relutrigine and vormatrigine are sodium channel modulators built to act on the hyperexcitable state of the channel while sparing normal function. Elsunersen is an antisense drug designed to lower SCN2A gene expression in a rare childhood epilepsy.

The evidence differs by program. The ulixacaltamide application rests on two positive studies in the Essential3 phase 3 program, and the relutrigine application on the registrational cohort of EMBOLD. Elsunersen has nine children in a sham-controlled study with a 77% sham-adjusted seizure reduction (EMBRAVE Part A). Vormatrigine's POWER1 trial missed its primary endpoint but met the 50% responder secondary endpoint.

The odds

Praxis says mid-cycle meetings for both applications found no major safety or efficacy concerns to date, that no advisory committee is planned and that FDA inspections ended with no Form 483. Target dates are Dec. 27, 2026 for relutrigine and Jan. 29, 2027 for ulixacaltamide. Across the industry, 90.6% of applications filed with the FDA are approved, and 93.6% in rare disease, per BIO, Informa and QLS data for 2011 to 2020. Those are background rates, not a forecast, and relutrigine's review has already been extended after the FDA treated added sensitivity analyses as a major amendment.

For vormatrigine, the same data put the chance of moving from phase 3 to a filing at 57.8%, though POWER1 was a phase 2/3 study. Praxis plans to restart POWER2 and start POWER3 in the fourth quarter. Elsunersen's registrational EMBRAVE3 trial is a single-arm study of about 30 patients with a 2027 readout.

The money

Praxis held about $1.4 billion at June 30, up from $926.1 million at the end of 2025, mainly because of a January offering of 2,543,800 shares at $260.00 that raised about $621.2 million net. It says that funds operations into 2028. The quarterly net loss was $83.7 million, and general and administrative expense rose to $27.5 million from $13.1 million a year earlier, which Praxis attributed mainly to professional expenses and personnel costs. The accumulated deficit was $1.3 billion. Under a 2024 license with Tenacia Biotechnology, Praxis is also eligible for $264.0 million in success-based milestones plus tiered royalties.

The risks

Concentration comes first. Two decisions about five weeks apart will decide whether Praxis becomes a commercial company, and the company has said it will not give regulatory updates until the PDUFA dates. The second risk is the epilepsy franchise. Vormatrigine targets common adult epilepsies, which Praxis puts at an estimated 3.5 million people in the U.S., and its next steps are still being designed. Third, a launch costs money before it earns any: Praxis has no product revenue and is hiring a field force. Finally, the mid-cycle account comes from the company, which has not published the FDA's own words.

Our read

The facts since June are better than the vormatrigine miss alone would suggest. On the company's account, the two applications that matter most had no major concerns at mid-cycle, the inspections were clean, and the money is in the bank. What is left is a binary run-up with a visible calendar: EMERALD in the fourth quarter, then Dec. 27, then Jan. 29. A relutrigine approval would depend on how the FDA reads the added analyses, and the company's silence until then means investors will learn the outcome from the agency. Vormatrigine is the open question for the longer term, because its restart will show whether the higher-dose signal in POWER1 holds up in a trial designed around it.
