Krystal Biotech · Opinion
Krystal Biotech has a profitable product and an unproven platform beyond skin
Vyjuvek earned $119.2 million in the second quarter, and the company holds about $1.1 billion. The next test is the KB803 readout due in the fourth quarter.
The technology
Krystal Biotech builds gene therapies on herpes simplex virus type 1 (HSV-1) vectors. KB707, for example, is a replication-defective HSV-1 immunotherapy, and the company's preclinical respiratory candidates KB409 and KB410 are also HSV-1 based. Vyjuvek, the company's only marketed product, delivers the COL7A1 gene to people with dystrophic epidermolysis bullosa. It can be redosed and given at home, per the UK approval release.
KB407 is an inhaled therapy for cystic fibrosis. KB707 encodes IL-12 and IL-2 and is being tested inhaled for lung cancer and by injection into skin tumors. KB803 and KB801 are aimed at eye conditions, per the second-quarter release. The October KB113 pilot used a low dose of intratumoral KB707 in three Gorlin syndrome patients.
The odds
Vyjuvek is approved in four regions, so the odds that matter now are for the pipeline. Rare disease programs fare better than most: the BIO, Informa and QLS data show 60.4% of rare disease programs move from phase 3 to filing and 93.6% of filings reach approval, with a 17.0% likelihood of approval from phase 1. For all diseases, only 28.9% of programs move from phase 2 to phase 3.
KB803 in corneal abrasions in dystrophic epidermolysis bullosa is the nearest registrational readout, with top-line data expected in the fourth quarter of 2026. The cystic fibrosis and lung cancer programs are earlier. KB407 has shown the vector reaches airway cells but no lung function data so far, per GlobalData. KB707 has early response rates of 27% to 36% in a small group of lung cancer patients, per CancerNetwork.
The money
Krystal is profitable. Second-quarter net income was $54.8 million on $119.2 million of Vyjuvek revenue, with a 95% gross margin. Cash, cash equivalents and investments were about $1.1 billion at June 30. The company guided 2026 non-GAAP R&D and SG&A to $175 million to $195 million, excluding stock-based compensation.
With spending of that size against $1.1 billion in cash and a business that earns money, the company does not need outside capital on these numbers. The release gave no runway figure, so this is our reading.
Growth is slowing, though. Revenue was up 24% year over year in the second quarter after rising 32% in the first. International launches in Germany, France and Japan add to sales, but pricing talks in Germany and France continue.
The risks
Vyjuvek is the only product with reported revenue. Vyjuvek treats a rare disease with a limited patient pool, and the slowing growth rate suggests the US launch is maturing. The company's second-quarter release shows cumulative sales of $965.9 million since launch.
International sales depend on pricing agreements. Germany talks run through at least the second half of 2026 and France into 2027, per the second-quarter release. The UK approval in May has no launch date until reimbursement procedures finish.
Pipeline data are early and small. The KB113 pilot had three patients and no control group. The KB407 cohort had seven patients. Timelines have also moved: GlobalData cited a registrational study enrolling in the second quarter of 2026, and the company's August release put a registrational start in 2027.
Our read
Krystal is an unusual small-cap in that it has an approved gene therapy, a profit, and a large balance sheet. That gives it time to run several expensive programs. What it does not yet have is a second product.
The next 12 months will show whether the approach works outside skin. KB803 is the first test with a registrational readout. The lung programs and KB407 will take longer. Investors should weigh the quality of the Vyjuvek business against how little is known about the other programs.
The next dated event is the third-quarter report on Nov. 2.
