Acadia's products carry the quarter while its pipeline story gets harder

Revenue guidance rose in August and cash reached $956.5 million. Then the lead pipeline drug narrowly missed its phase 2 primary endpoint.

The technology

Acadia has two marketed drugs and one lead development program. Nuplazid (pimavanserin) is, according to Acadia, the first and only FDA-approved drug for Parkinson's disease psychosis. Daybue (trofinetide) is built on a molecule derived from insulin-like growth factor 1 and treats Rett syndrome. The development bet is remlifanserin, a selective 5-HT2A inverse agonist meant to treat hallucinations and delusions in Alzheimer's disease psychosis without blocking dopamine D2 receptors.

The odds

The next test is remlifanserin. On Sept 24 the phase 2 portion of the RADIANT program missed its primary endpoint at p=0.0603 for the 60 mg dose, though a clinical global impression measure reached nominal significance. Acadia kept its two phase 3 studies running at 60 mg. Citi cut its success estimate to 20% from 35%.

The BIO/Informa/QLS 2011 to 2020 study reports that 28.9% of phase 2 programs advance to phase 3, 57.8% of phase 3 programs reach a regulatory filing, and 52.4% of drugs that enter phase 3 are eventually approved. A drug that goes into phase 3 after a near miss may sit below those averages, but the study does not give that number.

The commercial products carry little clinical risk. They are approved, and the European Commission added Daybu in August. Their risks are commercial: patent life, pricing and the cost of selling them. The releases reviewed give no phase 3 readout date for remlifanserin, so the next pipeline information is the full phase 2 dataset in November.

The money

Acadia is profitable. Second-quarter revenue was $308.0 million, net income was $31.5 million, and cash and investments were $956.5 million at June 30. Nuplazid sold $183.2 million and Daybue $124.8 million. The company raised its 2026 revenue guidance to $1.24 billion to $1.30 billion, and its stated ambition is about $1.7 billion of annual net sales in 2028. It gave no runway figure.

Spending is rising. SG&A was $160.3 million in the quarter against $133.5 million a year earlier, and 2026 SG&A guidance is $660 million to $700 million. R&D guidance was lowered to $355 million to $380 million from $385 million to $410 million, a cut the release does not explain.

The risks

The first is the pipeline. The Sept 24 result left the lead program at p=0.06 and a modest 0.26 effect size on its primary measure. Phase 3 has to do better with a similar population and the 30 mg arm removed.

The second is Nuplazid's patent life. A June 2025 Federal Circuit ruling affirmed the composition of matter patent, which protects the drug into 2030. A formulation patent for the 34 mg capsule runs into 2038.

The third is Daybue's growth and its price. The product is growing 30%, and the new weight-banded dosing patent would extend U.S. protection to March 2041 if it issues. In Europe, the company still has to negotiate pricing country by country, and the release gives no date.

Our read

Acadia is a cash-generating company whose valuation has depended on a pipeline asset that just got harder to believe. The business without remlifanserin is solid: two products, rising guidance, $956.5 million of cash, and a patent extension for Daybue that is allowed but not issued.

The question for investors is how much of the pipeline value survives the RADIANT result. The full data at the CTAD conference on Nov 16 to 19 will say more about the effect on individual symptoms and time course. Until then, the second-quarter report is the clearest guide to what the company earns, and the third-quarter figures will show whether Daybue's growth is holding.
