Oragenics, Inc. (NYSE American: OGEN) said on April 13, 2026 that it has dosed the first patient in its ongoing Phase IIa clinical trial of ONP-002, the company's lead candidate for concussion, also called mild traumatic brain injury (mTBI). The patient was treated at Mackay Hospital in Australia, the first site activated in the trial. The company said Mackay was activated on March 31, 2026, and that the first patient was dosed within days of that activation.

The company describes concussion as the most prominent neurological condition without an FDA-approved pharmacological treatment. Citing the CDC, it says an estimated 1.7 to 3.8 million people in the U.S. experience traumatic brain injuries annually, and it puts the global figure at 69 million. Oragenics said that if the FDA approves ONP-002, the drug would be the first and only pharmacological standard of care for a concussion market that it projects to reach over $9 billion by 2030. That is the company's projection, and the drug is years from any approval.

The trial design

The Phase IIa study is randomized and placebo-controlled. It is designed to evaluate 40 patients who meet enrollment criteria based on CT scan findings, presenting symptoms, and emergency room or hospital admission. Patients receive their first dose within 12 hours of a concussion and continue treatment for up to 30 days. Follow-up visits assess safety and tolerability through nasal examinations, physical assessments and neurocognitive testing.

Two more Australian sites, Alfred Hospital in Melbourne and Royal Adelaide Hospital in Adelaide, are progressing through site governance approvals, and the company expects them to activate in the second quarter of 2026. Southern Star Research, an Australian contract research organization, is managing the trial operations. Sterling Pharma Solutions is providing cGMP drug manufacturing from its facility in Cary, North Carolina.

Oragenics expects the Phase IIa findings to support its planned investigational new drug (IND) application to the FDA, which it targets for Q4 2026.

What the drug is

The company's 10-K business section describes Oragenics as a development-stage biopharmaceutical company focused on nasal delivery therapies for neurological conditions and infectious diseases. The filing calls ONP-002 a fully synthetic, non-naturally occurring neurosteroid that the company believes can cross the blood-brain barrier. The drug is manufactured as a powder and filled into a breath-propelled intranasal device. The company says the device closes the soft palate and keeps the drug in the nasal cavity.

The April release says the Phase 1 trial, in 40 patients, reported zero serious adverse events across all dose levels, and that preclinical data showed reductions in swelling, inflammation and oxidative stress in the brain, along with improvements in functional recovery. The release gives no efficacy data from people, and the Phase 1 safety finding comes from the company's own description. The 10-K lists an issued U.S. patent that expires 9/17/2040 with a maximum five-year patent term extension, and a second issued U.S. patent that expires 9/17/2035 without an extension.

Financial position

The XBRL data from the 10-K filed March 16, 2026 shows research and development expense of $2.4 million for 2025, down from $4.1 million for 2024. The net loss was negative $9.8 million for 2025, compared with negative $10.6 million for 2024. Cash from operations was negative $9.2 million for 2025 and negative $8.6 million for 2024. Year-end 2024 cash and cash equivalents were $864,840.

What comes next

Oragenics said the trial is now underway, but one patient has been dosed out of the 40 planned, and the release reports no results. Cash from operations was negative $9.2 million for 2025 against year-end 2024 cash of $864,840. Its stated next steps are the Alfred Hospital and Royal Adelaide Hospital activations in the second quarter of 2026, the IND submission targeted for Q4 2026, and U.S. Phase IIb trials to follow once the FDA approves the IND. The 10-K timeline estimates a U.S.-based clinical trial start in Q1 2027 and a Phase 3 start in Q1 2028, and calls the plan an estimate that is subject to funding, technical risks and regulatory approvals.

The release carries standard forward-looking statement language, warning that the timing and results of the trials could differ from expectations.
