Alaunos Therapeutics, Inc. (Nasdaq: TCRT) announced updated preclinical data on May 18, 2026 for ALN1003, its investigational oral metabolic candidate. The data come from non-Good Laboratory Practice (non-GLP) diet-induced obesity (DIO) mouse studies and cover insulin resistance biomarkers, adipose endocrine signaling and qualitative liver histology. The company said the findings are early and preclinical, and that they support continued preclinical development of the compound in metabolic syndrome and related conditions.

The studies behind the update

The 48-day study, DIO Study 1, gave ALN1003 orally in low, controlled doses split into two each day. There were 12 mice in each group, housed three to a cage. The 18-day study, DIO Study 2, was a pilot that gave ALN1003 in drinking water at three dose levels, with 6 mice in each group housed two to a cage. Treatment lasted 14 days, followed by a 4-day pharmacokinetic period.

Alaunos describes itself as a preclinical-stage biopharmaceutical company focused on orally administered small-molecule therapeutics for obesity and related metabolic disorders, including MASLD. The 10-K for 2025 says the company was previously a clinical-stage, oncology-focused cell therapy company developing adoptive TCR-T cell therapies. It has not generated any product revenue and has reported significant net losses every year since inception, with an accumulated deficit of $924.6 million at December 31, 2025.

Insulin resistance, adipose signaling and the liver

In the longer 48-day model, ALN1003 was associated with significantly lower fasting insulin and significantly lower Homeostasis Model Assessment of Insulin Resistance (HOMA-IR), a calculated index derived from fasting glucose and insulin. The release says the HOMA-IR difference held after adjustment for percentage body fat in a standard ANCOVA analysis (p=.0006), and with heteroscedasticity-robust HC3 standard errors as a sensitivity analysis (p=0.0014). ALN1003 was also associated with numerically lower leptin, significantly higher adiponectin and a significantly higher adiponectin-to-leptin ratio.

Liver histology findings provided additional support, according to the company. In a blinded pathology review, the company evaluated three control and three treatment samples from each study, and selected the high-dose group from the 18-day study. Control samples in that set had NAS scores of 5. Treated samples had lower mean NAS scores, 2.7 in the 48-day study and 1.3 in the 18-day study. Alaunos said these limited findings do not establish MASLD resolution, fibrosis reversal, inflammation improvement or clinical efficacy, but are consistent with a qualitative anti-steatotic effect in this preclinical model.

The pathology sample is tiny: three control and three treated livers from each study, with the high-dose group chosen from the 18-day study. The release also frames the market for the data. Alaunos says the treatment landscape is moving beyond weight loss alone, and believes ALN1003's early preclinical profile may align with that direction. That is the company's view, and the release names no competing programs.

Limits, cash and funding

The release lists limited sample sizes, histology limited to a sample of available livers, single-timepoint biomarker assessments, known constraints of HOMA-IR interpretation in rodent models, and qualitative or semi-quantitative pathology scoring. It states that ALN1003 has not been evaluated in human clinical trials.

Holger Weis, chief executive officer, said: "We are encouraged by the consistency of signals across insulin-resistance-related biomarkers, adipose endocrine markers, and liver histology while acknowledging these findings are early and preclinical." He added that the data support continued investigation of ALN1003 as a potential oral, non-hormonal approach.

The company plans additional preclinical studies focused on dose optimization, exposure-response relationships, expanded metabolic phenotyping and further characterization of liver pathology and underlying mechanisms.

The release reports cash and cash equivalents of approximately $0.354 million as of March 31, 2026. It says the current cash runway extends into the second quarter of 2026 and that the company intends to pursue additional financing to support continued operations. In plain terms, the cash was close to running out, and any financing will likely dilute holders. The 10-K reported a net loss of $4.2 million for 2025, compared with $4.7 million for 2024, and research and development expense of $1.4 million, up from $362,000.
