Q32 Bio Inc. presented results from Part B of its SIGNAL-AA Phase 2a trial of bempikibart in alopecia areata in a late-breaking session at the European Academy of Dermatology and Venereology Congress 2026. The company announced the findings on September 30, 2026. It said the Week 36 data showed clinically meaningful efficacy on the primary and key secondary endpoints, and that the response was durable through a 16-week off-drug period.

## The Part B results

Part B is an open-label trial with no placebo group, in 33 patients with severe or very severe alopecia areata, with baseline SALT scores of 50 to 100 and a maximum duration of current episode of four years. Prior JAK inhibitor exposure was allowed, and 36.4% of the 33 enrolled patients had been treated with an oral JAK inhibitor.

Patients received bempikibart subcutaneously: 200mg weekly for four doses, then 200mg every other week for 32 weeks.

At Week 36, the mean percent reduction in SALT score from baseline was 35.3% in the modified intent-to-treat analysis. In that population, 40.0% (10/25) of patients achieved a SALT-20 response, which means 80% scalp hair coverage. In the intent-to-treat analysis of all enrolled patients, 30.3% (10/33) achieved a SALT-20 response. SALT 30 responses were 44.0% (11/25) in the mITT population and 33.3% (11/33) in the ITT population. SALT 50 responses were also 44.0% (11/25) and 33.3% (11/33).

Ten patients in the mITT analysis had received an oral JAK inhibitor before enrolling. Four of them had responded to their most recent JAK inhibitor, defined as at least a 30% SALT improvement, and six had not. The mean percent reduction in SALT score was 48.3% in prior JAK responders and 4.8% in prior JAK non-responders; JAK-naive patients had a mean reduction of 44.1%.

## The off-drug period

In total, 18 patients entered the 16-week off-drug period. At the 16-week point, 44% of them achieved a SALT-20 response, 61% achieved a SALT 30 response and 50% achieved a SALT 50 response.

Of the 18, twelve had shown hair growth by Week 36. Maintenance of response, defined as less than a 20-point SALT worsening from the last on-treatment value, was observed in 100% of those patients, and deepening of response was seen in 42%. One patient who reached SALT-10 at Week 36 achieved complete hair growth, SALT 0, during the off-drug period.

Dr. José Trevejo, Chief Medical Officer, said "We believe these data support a less frequent, chronic maintenance dosing regimen once patients respond."

## Safety, pharmacology and what comes next

The company said bempikibart was generally well tolerated, with no new safety signals, a general statement that comes without event counts in the figures presented. Pharmacokinetic data showed the loading regimen reached steady state concentrations approximately 10 weeks earlier than in Part A. Expected reductions in CD3+ T-cells were observed.

Q32 Bio said it announced topline results in July 2026 and intends to advance a registration-directed program in the first half of 2027. That program has not started.

## What the data do and do not show

The headline is a mean 35.3% reduction in SALT score. SALT-20 was reached by 40.0% (10/25) of the mITT group but by 30.3% (10/33) of all enrolled patients. The results come with no p-values and no control arm, so they cannot separate drug effect from chance or from natural hair regrowth. The six prior JAK non-responders averaged a 4.8% reduction, so benefit was concentrated in other patients. The durability claim rests on 12 patients with hair growth at Week 36, out of 18 who entered the off-drug period.
