Inmagene Biopharmaceuticals, a privately held company that is to merge into Ikena Oncology, announced on July 1, 2025 that it had dosed the first patient in its Phase 2b ADAPTIVE trial of IMG-007 in moderate-to-severe atopic dermatitis. Ikena filed the release with the SEC as an exhibit to a Form 8-K, and it described the release as merger material. The release gives no efficacy data from the trial.

## The merger

Ikena, two Cayman Islands merger subsidiaries it wholly owns, and Inmagene signed an Agreement and Plan of Merger on December 23, 2024. One subsidiary would merge into Inmagene, which would survive as a wholly owned subsidiary of Ikena, and Inmagene would then merge into the second subsidiary. The closing depends on conditions in the merger agreement, including a concurrent financing and shareholder approval.

Ikena's registration statement on Form S-4, File No. 333-285881, was first filed on March 18, 2025 and declared effective on June 11, 2025. The registration statement contains the proxy statement and prospectus for Ikena's stockholders. Ikena's own risk list names the failure to obtain shareholder approval in time as a risk to closing.

Ikena's 10-K describes it as a targeted oncology company with a limited operating history, and says it has incurred significant net losses since its inception. It also says that if the merger or another strategic transaction is not completed, the board may decide to dissolve and liquidate the company, and that the cash available to shareholders would depend on the timing and on reserves for commitments and contingent liabilities.

## The ADAPTIVE trial

The ADAPTIVE trial, identified in the release as NCT07037901, is a randomized, double-blind, placebo-controlled Phase 2b study of several subcutaneous dose regimens of IMG-007 in adults with moderate-to-severe atopic dermatitis who have not responded to or cannot tolerate topical therapy. It aims to enroll about 220 patients across four arms in a 1:1:1:1 ratio: high, medium and low doses of IMG-007 and placebo.

The trial has two periods. The first is a 20-week randomized, placebo-controlled treatment period. The second is a 32-week active treatment period in which all participants, including those first given placebo, receive IMG-007. The primary endpoint is the mean percentage change from baseline in the Eczema Area and Severity Index at week 20. Key secondary endpoints include the same change at week 16, the proportion of patients reaching EASI-75 at week 16 and the proportion scoring 0 or 1 on the Investigator's Global Assessment at week 20. Topline results are expected in the fourth quarter of 2026, and the company says the study is meant to guide dosing for future Phase 3 trials.

IMG-007 is a humanized, subcutaneous, non-depleting IgG1 antibody against OX40. The release says it was originally discovered by HUTCHMED, and that its subcutaneous formulation has a half-life of 34.7 days at the expected therapeutic dose. The release also says a Phase 2a trial in moderate-to-severe atopic dermatitis showed sustained clinical activity and was well tolerated, with no reported cases of pyrexia or chills. Those are the company's descriptions. The release gives no response rates, sample sizes or safety counts for that earlier study.

## What the release does not show

The first patient dosed is the only trial event in the release. No placebo-controlled results exist yet, and the primary readout is not expected until the fourth quarter of 2026, so Ikena stockholders voting on the merger will not have the trial's main data. The release says the company's lead asset is in Phase 2 for both atopic dermatitis and alopecia areata, and that its IMG-004, an oral BTK inhibitor with an extended half-life, is ready to advance into Phase 2 development.

The release calls IMG-007 a potentially differentiated candidate. It does not compare IMG-007 with any approved or late-stage atopic dermatitis therapy, and it does not say what a Phase 3 program would cost or when it would begin. Those points are open in the filings.
