A Genprex-supported abstract at the 2026 ASCO Annual Meeting linked Trop-2 and PTEN protein levels to progression-free survival in non-small cell lung cancer, but not in small cell lung cancer.

Genprex, Inc. (Nasdaq: [GNPX](https://aibio.space/companies/genprex/)) issued a press release on May 26, 2026 announcing that research collaborators had an abstract published at the 2026 American Society of Clinical Oncology Annual Meeting. The abstract, titled "Predictive biomarkers for PFS in patients receiving quaratusugene ozeplasmid," reports clinical data on predictive biomarkers in patients receiving REQORSA Gene Therapy for lung cancer. The company disclosed the release in a Form 8-K filed the same day.

## The clinical data

The company said the data came from patients in its Acclaim clinical trials. Quaratusugene ozeplasmid is a gene therapy that delivers a plasmid coding for the TUSC2 tumor suppressor gene to lung cancer cells. The filing notes that greater than 80% of lung cancers have decreased or absent TUSC2 protein, but TUSC2 protein levels themselves did not correlate with progression-free survival.

Earlier preclinical work had found higher levels of Trop-2 protein in organoids and lower levels of PTEN protein in lung cancer cell lines correlated with response. Tumor tissue from patients in three clinical trials was tested for both proteins using immunohistochemistry on archival samples, with results expressed as H-scores. Data on both proteins and on progression-free survival were available for 18 patients. Six had non-small cell lung cancer and were enrolled in Acclaim-1 with osimertinib. One was enrolled in Acclaim-2 with pembrolizumab. Eleven had small cell lung cancer and were enrolled in Acclaim-3 with atezolizumab.

## What the abstract found

In patients with non-small cell lung cancer, Trop-2 H-scores above 100 correlated with prolonged progression-free survival (p=0.05). PTEN H-scores below 100 correlated with prolonged progression-free survival (p=0.03). In patients with small cell lung cancer, Trop-2 H-scores were universally low and non-evaluable. PTEN H-scores in that group did not correlate with prolonged progression-free survival (p=0.53). The filing summarizes the conclusion this way: both Trop-2 H-scores above 100 and PTEN H-scores below 100 correlated with longer progression-free survival in non-small cell lung cancer, but not in small cell lung cancer.

Genprex also analyzed non-small cell lung cancers with the highest staining intensity. Tumors with 3+ Trop-2 staining had a strong relationship with progression-free survival that was just outside the bounds for significance (p=0.053). Tumors with 3+ PTEN staining showed a trend for a negative relationship that was not statistically significant (p=0.309). The company said these results are consistent with the H-score analysis.

## The 10-K background and finances

The 10-K business section says that because not all patients respond to REQORSA, the company has been working with researchers at MD Anderson to identify biomarkers that might predict response in lung cancer. That work identified two proteins whose expression appears to predict response, and validation in clinical trial specimens is still needed. The 10-K says the Acclaim-1 trial is enrolling in its Phase 2a expansion portion.

For the fiscal year ended December 31, 2025, the XBRL data show research and development expense of $9.4 million and a net loss of $16.2 million. For 2024, the comparable figures were $10.5 million and $21.1 million. Cash from operations was negative $15.3 million in 2025 and negative $17.9 million in 2024. Year-end cash and cash equivalents for 2024 were $1.6 million.

## What comes next

The company said high (3+) Trop-2 expression will be investigated further as a potential biomarker for REQORSA. The press release said the identified biomarkers may predict patient response, enable targeted patient selection and allow for precision medicine. The filing does not give dates for that further work.

The numbers are thin. The significant findings in non-small cell lung cancer rest on seven patients, with p-values of 0.05 and 0.03, and the strongest-staining Trop-2 result at p=0.053 fell just outside significance. The samples were archival, the 18 patients came from three different trials with three different partner drugs, and no control group is described, so the abstract cannot show that the biomarkers predict response to REQORSA rather than the prognosis of the patients. In small cell lung cancer, Trop-2 was non-evaluable and PTEN did not correlate. TUSC2, the protein REQORSA delivers, did not correlate with progression-free survival at all. The 10-K itself says validation in clinical trial specimens is still needed. The company reported a 2025 net loss of $16.2 million.
