## The FDA decision

Fulcrum Therapeutics, Inc. (Nasdaq: [FULC](https://aibio.space/companies/fulcrum-therapeutics/)) said on June 1, 2026 that it is discontinuing pociredir, its lead clinical program for sickle cell disease (SCD), and starting a comprehensive review of strategic alternatives. The decision followed meeting minutes that Fulcrum received on May 28, 2026 from recent end-of-phase interactions with the FDA. The minutes reflected heightened FDA concerns about pociredir's benefit-risk profile in SCD.

The release ties those concerns to Tazverik (tazemetostat), another PRC2 inhibitor. It says Tazverik was linked to an unexpectedly high rate of secondary hematologic malignancies and was withdrawn from the global market in March 2026. Fulcrum argued that its target, EED, and Tazverik's target, EZH2, perform different biological roles. The FDA considered that argument and concluded that any intervention targeting the PRC2 complex carries equivalent malignancy risk, whichever subunit it engages. The release says the agency's position is informed by pociredir's previously disclosed preclinical malignancy observations, and that it left no viable regulatory path forward.

Chief Executive Officer Alex C. Sapir said the company made "the very difficult decision to discontinue development of pociredir." He also said that "no new safety signals have been observed to date with pociredir." Sapir acknowledged the fetal hemoglobin elevations and the potential for clinical benefit, but said the company does not see a path forward with the drug.

## What the early data show

Pociredir is an oral small-molecule inhibitor of Embryonic Ectoderm Development, or EED. The release says that inhibiting EED reduces fetal globin repressors, including BCL11A, and raises fetal hemoglobin (HbF). In the PIONEER Phase 1b trial in people with SCD, the company reports dose-dependent increases in HbF and improvements in markers of hemolysis and anemia. Across the 12 mg and 20 mg cohorts, the drug was generally well tolerated, with up to three months of exposure and no treatment-related serious adverse events reported. The FDA had granted pociredir Fast Track and Orphan Drug Designation for SCD.

These findings come from the company's own announcement. The release does not say how many patients were treated in each cohort, and its safety statement covers at most three months of exposure. The FDA's objection is not a trial safety signal. It rests on the class risk the agency drew from Tazverik, which the release says is informed by pociredir's preclinical observations.

## The disease and the company

SCD is a genetic disorder of the red blood cells caused by a mutation in the HBB gene. The release says the disease can cause anemia, pain, infections, stroke, heart disease, kidney failure and reduced life expectancy. Sapir said the SCD community has faced many disappointments and setbacks in developing new treatments.

Fulcrum describes itself as a company that uses proprietary technology to find drug targets that modulate gene expression. With pociredir discontinued, the release does not name another clinical program, and it does not say what, if anything, remains in development.

## The strategic review

Fulcrum said it will explore alternatives including, but not limited to, a merger, acquisition, business combination or other strategic transaction involving the company or its assets. It has not set a timeline. The company does not intend to provide further updates unless the board approves a course of action, the review ends, or other disclosure is determined to be appropriate.

The release says the company has begun efforts to significantly reduce operating expenses and preserve capital. It gives no figures for those cuts, for headcount or for the cost of a wind-down.

## The cash position

Fulcrum had $333.3 million in cash, cash equivalents and marketable securities as of March 31, 2026, according to the release. No later balance appears in it. Its annual report, filed February 24, 2026, shows a net loss of $74.9 million for 2025, cash used in operations of $60.1 million and research and development expense of $56.1 million.

The release gives no estimate of the cash that the review or the wind-down will use, and it does not say whether the company plans to return cash to stockholders. With no clinical program named, that balance is the main asset the strategic review has to work with.
