Fractyl Health said on October 22, 2026, that it will cut [about 45% of its workforce](https://www.biopharmawatch.com/news/GUTS/fractyl-health-revital-focus-fda-submission) and stop internal development of Rejuva, its gene therapy platform, to focus on Revita, a one-time endoscopic procedure meant to help people keep weight off after they stop GLP-1 drugs.

The Burlington, Massachusetts company expects the cuts to be [substantially complete by November 2026](https://www.biopharmawatch.com/news/GUTS/fractyl-health-revital-focus-fda-submission). It said quarterly operating cash burn should fall about 50%, to roughly $8 million, by the end of the second quarter of 2027, and that existing cash will fund operations into the second quarter of 2027.

## The details

Fractyl is seeking [strategic alternatives for Rejuva](https://www.biopharmawatch.com/news/GUTS/fractyl-health-revital-focus-fda-submission) and said there is no assurance that a transaction will result. Rejuva's lead candidate, RJVA-001, is a one-time AAV gene therapy for type 2 diabetes that had received [authorization in the Netherlands](https://www.biopharmawatch.com/news/GUTS/fractyl-health-first-in-human-trial-rjva-001) for a first-in-human phase 1/2 trial, announced in May.

Co-founder Jay Caplan will retire from his executive roles effective October 30, 2026, and stay on as a senior adviser, [the company said](https://www.biopharmawatch.com/news/GUTS/fractyl-health-revital-focus-fda-submission).

"We expect the capital we have today to be sufficient to fund our planned De Novo submission," said Harith Rajagopalan, co-founder and CEO, [in the release](https://www.biopharmawatch.com/news/GUTS/fractyl-health-revital-focus-fda-submission).

The plan keeps Revita's timeline. Fractyl still expects to file a De Novo request with FDA in late fourth quarter 2026, report 12-month registrational data in the first quarter of 2027 and, if authorized, launch in the U.S. in [early 2028](https://www.biopharmawatch.com/news/GUTS/fractyl-health-revital-focus-fda-submission).

## Context

The restructuring came six days after Fractyl reported that the [REMAIN-1 registrational cohort](https://www.biopharmawatch.com/news/GUTS/fractyl-health-revita-remains-1-study-results) met its six-month co-primary endpoint. In 308 patients who lost at least 15% of body weight on tirzepatide and then stopped, Revita patients regained 5.1% of body weight against 8.2% for sham (p=0.0023). Shares fell sharply that day, according to [BiopharmaWatch market data](https://www.biopharmawatch.com/news/GUTS/fractyl-health-revita-remains-1-study-results).

Fractyl had been trimming spending all year. It reported [$63.2 million in cash](https://www.biopharmawatch.com/news/GUTS/fractyl-health-q1-2026-financial-results) at March 31, down from $81.5 million at the end of 2025, with first-quarter R&D of $15.6 million. In May it guided to a cash runway into early 2027. In June it hired [Mike Zumdahl](https://www.biopharmawatch.com/news/GUTS/fractyl-health-appoints-mike-zumdahl-as-senior-vice-president-market-access-and-commercial-guts-20260601), a former Novo Nordisk and Inari Medical market access executive, to lead market access and commercial strategy.

## What it means for investors

By the company's own guidance, the cuts add about one quarter of runway, from [early 2027](https://www.biopharmawatch.com/news/GUTS/fractyl-health-q1-2026-financial-results) to the second quarter of 2027. That covers the De Novo filing and the 12-month data. It does not cover an FDA decision, [which the company targets for late 2027](https://www.biopharmawatch.com/news/GUTS/fractyl-health-revital-focus-fda-submission), or a launch in 2028.

That gap means Fractyl will likely need new money, a partner or both before Revita can earn revenue. Its [12-month data](https://www.biopharmawatch.com/news/GUTS/fractyl-health-revita-remains-1-study-results) arrive in the first quarter of 2027. A raise after positive 12-month data would probably come on better terms than one before, which makes the first-quarter readout the next swing point.

Dropping Rejuva removes the company's second shot. Fractyl is now a single-product company whose value rides on one regulatory filing and one remaining co-primary endpoint: whether more than 50% of Revita patients keep at least 5% weight loss at 12 months, measured against an [FDA-mandated performance goal](https://www.biopharmawatch.com/news/GUTS/fractyl-health-revita-remains-1-study-results).

## What to watch

Workforce cuts finish by November 2026. The De Novo submission is planned for late fourth quarter 2026 and 12-month data for the first quarter of 2027. A Category III CPT code for the procedure takes effect on [January 1, 2027](https://www.biopharmawatch.com/news/GUTS/fractyl-health-revita-remains-1-study-results).
