# Xenon has an epilepsy filing and a depression problem

Azetukalner's NDA is in and the cash lasts into 2029. A pause in the depression trials after psychosis reports cut the shares sharply and left psychiatry as the open question.

## The technology

Azetukalner is a once-daily pill that opens KV7 potassium channels in neurons, which makes overactive nerve cells less likely to fire. The same mechanism is why Xenon is testing it in epilepsy, depression and bipolar depression. The drug has a long exposure record in epilepsy: Xenon cites [more than 1,500 patient-years](https://finviz.com/news/393312/xenon-announces-azetukalner-nda-submission-to-fda-for-focal-seizures-and-provides-update-on-psychiatry-clinical-program) of data.

In the phase 3 X-TOLE2 study, the 25 mg dose cut median monthly focal seizures by [53.2% against 10.4% on placebo](https://www.sec.gov/Archives/edgar/data/1582313/000119312526097577/d107087dex991.htm) at week 12, a placebo-adjusted reduction of 42.7%. Open-label data presented at AAN showed a [90.9% median reduction at month 48](https://www.clinicaltrialvanguard.com/news/xenon-presents-azetukalner-phase-3-seizure-study-results/) among patients who stayed in the extension.

Xenon also has early NaV1.7 and KV7 pain programs in phase 1, plus NBI-921355, a NaV1.2/NaV1.6 inhibitor [partnered with Neurocrine](https://investor.xenon-pharma.com/node/14566/pdf).

## The odds

Epilepsy is the nearest decision. Xenon filed its NDA on [September 17](https://finviz.com/news/393312/xenon-announces-azetukalner-nda-submission-to-fda-for-focal-seizures-and-provides-update-on-psychiatry-clinical-program) on two positive efficacy studies. The BIO/Informa/QLS base rates show that [57.8% of phase 3 programs reach a filing and 90.6% of filings reach approval](https://go.bio.org/rs/490-EHZ-999/images/ClinicalDevelopmentSuccessRates2011_2020.pdf), and Xenon has cleared the first of those steps. The FDA has not yet been reported to have accepted the filing, and no action date has been published in the sources reviewed.

Depression is less certain. William Blair cut its probability of success in MDD from 60% to 50% after the enrollment pause, having already viewed the indication as risky because of [mixed phase 2 data](https://fiercebiotech.com/biotech/xenon-pauses-phase-3-depression-trials-after-psychosis-events). Across all areas, the BIO/Informa/QLS data put the likelihood of approval from phase 3 at [52.4%](https://go.bio.org/rs/490-EHZ-999/images/ClinicalDevelopmentSuccessRates2011_2020.pdf), a rough guide for a program whose safety profile is now under question.

## The money

Xenon had [$1,245.1 million in cash and securities at June 30](https://investor.xenon-pharma.com/node/14566/pdf) and said that funds operations into 2029. The balance was rebuilt in March, when the company [sold stock at $57 a share](https://www.sec.gov/Archives/edgar/data/1582313/000119312526102517/d83218d424b5.htm) in an offering that, with the at-the-market program, brought in a combined [$837.6 million of net proceeds](https://investor.xenon-pharma.com/node/14351) in the first quarter.

Spending is rising. The second-quarter net loss was $110.7 million, and research and development expense was $99.2 million, up from $75.0 million a year earlier. The company has no product revenue. Dilution has been material: weighted average shares rose from 78.9 million in the second quarter of 2025 to 99.6 million in the second quarter of 2026, [per the Q2 report](https://investor.xenon-pharma.com/node/14566/pdf).

## The risks

Two risks matter most. The first is safety in psychiatry. Xenon paused enrollment in its MDD and bipolar studies after neuropsychiatric events, including a very small number coded as psychosis, and the company is [evaluating dosing changes](https://fiercebiotech.com/biotech/xenon-pauses-phase-3-depression-trials-after-psychosis-events). Management says the events were mild to moderate, short and reversible, and they did not appear in the phase 2 depression study. A dosing change could weaken efficacy. The topline date moved earlier, to the first quarter of 2027, because Xenon stopped enrolling at about 80% of its target.

The second is tolerability in epilepsy. In X-TOLE2, dizziness was 20.5% against 3.2% on placebo, and adverse events led to [discontinuation in 14.5% of patients on 25 mg](https://www.sec.gov/Archives/edgar/data/1582313/000119312526097577/d107087dex991.htm). Reviewers and prescribers will weigh that against the efficacy. A smaller concern is the Kv7 class: Biohaven paused epilepsy trials of BHV-7000 a week before Xenon's pause, and Mortimer said nothing public suggests the two are linked.

## Our read

Xenon is an epilepsy company with a psychiatry option attached. The epilepsy data are strong, the filing is in, and the balance sheet carries the company well past a possible launch. Investors were paying for more than epilepsy: BioPharma Dive reported the market value had risen above $5 billion, and a pause in two studies took roughly a quarter off the shares in a day, per Fierce.

What to keep in mind is how little is known about the depression events. The company says they are reversible and rare, and William Blair says they resemble rates seen with common antidepressants. Neither claim can be checked until the X-NOVA2 data arrive. For epilepsy, the questions are narrower: whether the FDA accepts the filing, and how it treats a 14.5% discontinuation rate at the top dose. The next hard dates are X-NOVA2 topline data in the first quarter of 2027 and the FDA's filing decision.
