# Vaxcyte won its phase 3. Now it has to win the market

VAX-31 met every primary endpoint in OPUS-1, and Vaxcyte raised about $1.1 billion days later. A 2028 filing, two serotype gaps and two giant rivals stand between it and sales.

## The technology

Vaxcyte makes pneumococcal conjugate vaccines on a cell-free protein synthesis platform, XpressCF, [licensed from Sutro Biopharma](https://www.sec.gov/Archives/edgar/data/1649094/000164909426000045/a20260831-8xkxpressrelea.htm). Conjugate vaccines link bacterial sugars to a carrier protein. Adding more serotypes usually means more carrier protein, which can weaken the immune response to each one. Vaxcyte says its site-specific, [carrier-sparing method](https://www.sec.gov/Archives/edgar/data/1649094/000119312526413168/d21202dex991.htm) lets it pack in more serotypes without that loss.

OPUS-1 was the test. VAX-31 met [all prespecified primary endpoints](https://www.sec.gov/Archives/edgar/data/1649094/000119312526413168/d21202dex991.htm) against [Pfizer's Prevnar 20 and Merck's Capvaxive](https://www.sec.gov/Archives/edgar/data/1649094/000119312526413168/d21202dex991.htm) in 4,047 adults. The flaw: against Capvaxive alone, serotypes 3 and 12F missed the stricter 0.667 noninferiority bar and met only the historical 0.5 bar.

## The odds

The largest clinical risk is behind the company. Two descriptive adult phase 3 trials, OPUS-2 and OPUS-3, report in the [first half of 2027](https://www.sec.gov/Archives/edgar/data/1649094/000119312526413168/d21202dex991.htm), followed by a manufacturing consistency study and a BLA in the first half of 2028. Historically [90.6% of filed applications](https://go.bio.org/rs/490-EHZ-999/images/ClinicalDevelopmentSuccessRates2011_2020.pdf) are approved across all diseases, and the safety data so far showed no vaccine-related serious adverse events.

The harder question is the label and recommendations. Chief scientific officer James Wassil said the company expects the comparison with Capvaxive to be [a particular focus of FDA review](https://www.sec.gov/Archives/edgar/data/1649094/000119312526413168/d21202dex991.htm), because Capvaxive's unique serotypes cause more adult disease in the U.S. today. How vaccine advisers treat serotypes 3 and 12F will shape uptake.

## The money

Vaxcyte had [$2,507.7 million in cash, cash equivalents and investments at June 30, 2026](https://www.sec.gov/Archives/edgar/data/1649094/000164909426000041/pcvx-20260805xexx991.htm), then raised about [$544.3 million in equity and $558.7 million in convertible notes](https://www.sec.gov/Archives/edgar/data/1649094/000119312526419174/d150490d8k.htm) in October. It used [$513.1 million in operating cash in the first half](https://www.sec.gov/Archives/edgar/data/1649094/000164909426000043/pcvx-20260630.htm) and is paying for dedicated manufacturing at Lonza in Switzerland. It has no product revenue.

## The risks

The main risk is commercial. Vaxcyte says the market is [$8.5 billion and could reach about $12 billion by 2030](https://www.sec.gov/Archives/edgar/data/1649094/000119312526413168/d21202dex992.htm), but it will enter against Pfizer and Merck, two of the three companies behind those [reported sales](https://www.sec.gov/Archives/edgar/data/1649094/000119312526413168/d21202dex992.htm). A BLA in 2028 means any launch is at least two years away.

Manufacturing is the second risk. A 31-valent vaccine is complex to make at scale, and Vaxcyte depends on [Lonza for important components](https://www.sec.gov/Archives/edgar/data/1649094/000164909426000043/pcvx-20260630.htm). The new convertible notes add $575 million of debt due in 2032 if not converted.

## Our read

Vaxcyte did what it needed to in OPUS-1, and the platform claim now has phase 3 support. It also raised money at a good moment. The company's value now depends on execution over a long runway: two more phase 3 readouts, a manufacturing study, a filing in 2028, and then a fight for recommendations against two of the largest vaccine makers. The serotype 3 and 12F results are the one clinical detail that could cost it share in that fight.

There is also a structural point in Vaxcyte's favor. Pneumococcal vaccines compete largely on breadth of coverage, and the company says VAX-31 could add [coverage of 13% to 36% of invasive pneumococcal disease](https://www.sec.gov/Archives/edgar/data/1649094/000119312526413168/d21202dex991.htm) and 19% to 31% of pneumococcal pneumonia over current adult vaccines. If regulators and advisers accept that breadth as worth more than a small shortfall on two serotypes, the product has a clear selling point. If they weigh the two serotypes heavily, Vaxcyte may face a narrower recommendation. The new leadership hires, including a chief medical officer who worked on [Prevnar 13 and Prevnar 20 at Pfizer](https://www.sec.gov/Archives/edgar/data/1649094/000164909426000045/a20260831-8xkxpressrelea.htm), suggest the company knows where that argument will be won. The infant program, with [phase 2 data due by the end of the first half of 2027](https://www.sec.gov/Archives/edgar/data/1649094/000164909426000041/pcvx-20260805xexx991.htm), is the other source of upside, aimed at a pediatric market the company describes as [large, well-established and highly durable](https://www.sec.gov/Archives/edgar/data/1649094/000119312526413168/d21202dex992.htm).
