- **Technology**: **Commercial and proven.** Two AAV gene therapies are now FDA approved, and four products already bring in revenue.

- **Odds**: **Mixed record.** Two approvals landed, but two phase 3 trials failed in nine months. Rare disease programs reach approval 17.0% of the time from phase 1, per the BIO study.

- **Balance sheet**: **Helped by vouchers.** Cash was $436 million at June 30 with $413 million of voucher sales pending, against a $97 million quarterly operating burn.

- **Main risk**: **Launch execution.** Two high-priced gene therapies need payer and center uptake, and the late-stage pipeline is thin after Aspire.

- **Watch next**: **UX701 Wilson disease data in Q4 2026.** Also the Angelman program decision, expense cuts and voucher closings.

## Fact-check-o-meter

Some gaps

Ultragenyx reports setbacks plainly, but its August framing now sits beside a cost-cutting plan

Ultragenyx's filings describe the Angelman failure and the earlier setrusumab misses without spin, and its liquidity language is conventional (no going concern warning). The gaps are about timing and emphasis: the August release reaffirmed a 2027 profitability path about four weeks before a failed phase 3 and a plan for significant expense cuts, and the 10-Q keeps a positive reading of setrusumab after two missed primary endpoints. A securities class action over the setrusumab trials is pending.

1. Press release vs filing · weight medium 2027 profitability path reaffirmed, then expense cuts flagged four weeks later **The company said:** "Reaffirm 2026 financial guidance, including total revenue of $730 million to $760 million and combined R&D and SG&A expenses to be flat to slightly down versus 2025; remain on path to profitability in 2027" [source](https://www.sec.gov/Archives/edgar/data/1515673/000119312526332802/rare-ex99_1.htm) **The record says:** The Sept. 2 8-K says: "The Company will also assess its planned operations to define and implement significant expense reductions, while supporting its growing commercial business." The 10-Q says cash, cash equivalents and marketable securities were $436 million at June 30, 2026 and that capital resources "will be sufficient to fund our projected operating requirements for at least the next 12 months". [source](https://www.sec.gov/Archives/edgar/data/1515673/000119312526380165/rare-20260902.htm) **Our view:** Nothing here contradicts the filings, and no going concern doubt is raised. But the August guidance was set before Aspire read out, and the documents reviewed do not say whether the 2027 profitability target still stands.
2. Science · weight high Angelman phase 3 was randomized and sham controlled, and showed no efficacy signal **The company said:** "we are ready to launch two gene therapy products and are preparing for a pivotal Phase 3 GTX-102 data readout in Angelman syndrome" [source](https://www.sec.gov/Archives/edgar/data/1515673/000119312526332802/rare-ex99_1.htm) **The record says:** The Sept. 2 8-K says the study "did not achieve the primary endpoint of change from Baseline in Bayley-4 cognitive raw score nor the key secondary endpoint of net response in Multidomain Responder Index" and "There were no differences between the treated and control groups that could support efficacy". ClinicalTrials.gov lists 129 participants, quadruple masking and the same Bayley-4 primary endpoint at Day 338. [source](https://www.sec.gov/Archives/edgar/data/1515673/000119312526380165/rare-20260902.htm) **Our view:** The company reported the result in neutral terms, which is fair. A sham-controlled phase 3 found no efficacy signal, and the remaining Aurora study is described in the release as open-label, so it cannot supply a randomized comparison.
3. Science · weight medium Genglycos approval rests on a randomized phase 3 with 44 evaluable patients; the confirmatory controls are external **The record says:** The Aug. 19 8-K says approval is based on the "48-week randomized, double-blind, placebo-controlled Phase 3 GlucoGene study" that "treated 46 participants" with 44 in the mITT population (DTX401 n=20, placebo n=24), "showing a reduction in the cornstarch requirements in the treated group (p<0.001)". As a condition of accelerated approval, the company will follow "50 patients and 20 control patients", where the controls are patients "who sought commercial treatment but cannot be treated with GENGLYCOS due to the presence of anti-AAV8 antibodies". ClinicalTrials.gov lists the primary endpoint as percent change in daily cornstarch intake at Week 48. [source](https://www.sec.gov/Archives/edgar/data/1515673/000119312526357185/rare-20260819.htm) **Our view:** The approval study was randomized and placebo controlled. Cornstarch intake is a burden measure rather than a hard clinical outcome, and the post-marketing controls are not randomized. Antibody-positive patients may differ from treated ones, so the confirmatory data will be harder to interpret than a randomized study.
4. Science · weight medium 10-Q keeps a positive reading of setrusumab after two missed primary endpoints **The record says:** The 10-Q says the phase 3 Orbit and Cosmic studies "did not achieve their primary endpoint of reduction in annualized clinical fracture rate", then adds: "We believe the data across two global Phase 3 studies suggest that setrusumab has a meaningful effect on bone disease in OI." It adds that the company will "continue to engage regulatory agencies to determine the necessary data to support a regulatory filing and if there is a potential path forward for UX143." [source](https://www.sec.gov/Archives/edgar/data/1515673/000119312526333233/rare-20260630.htm) **Our view:** The miss is stated first and clearly. The 10-Q says the January data included bone mineral density, vertebral fractures and patient reported outcomes but does not say which measures support the belief, so readers should treat it as management's view and not a regulatory position.
5. Legal and disclosure record · weight medium Securities class action and derivative suit over the setrusumab trials **The record says:** The 10-Q's Legal Proceedings lists Steven Bailey v. Ultragenyx Pharmaceutical Inc., Emil D. Kakkis, and Eric Crombez, M.D., a putative class action filed in February 2026 in the U.S. District Court for the Northern District of California under Sections 10(b) and 20(a) of the Exchange Act. The complaint is described as "alleging that the Company made false and misleading statements about the design and prospects of the UX143 Orbit and Cosmic clinical trials." A putative derivative complaint was filed in the same court in March 2026. The 10-Q also lists Hatch-Waxman patent suits over Dojolvi; on Oct. 19 the company settled with Navinta (generic entry from Jan. 1, 2033), and similar suits against other filers remain pending. [source](https://www.sec.gov/Archives/edgar/data/1515673/000119312526333233/rare-20260630.htm) **Our view:** These are allegations in complaints, not findings. The 10-Q says the complaints seek unspecified damages and gives no loss estimate.
