# Travere's FILSPARI growth is real. The CEO change and pipeline add risk

U.S. sales of the kidney drug nearly doubled in the second quarter on the FSGS launch. A new chief executive arrives December 1, and the pegtibatinase trial reads out in 2027.

## The technology

Travere's main product is FILSPARI (sparsentan), which the company describes as a [dual endothelin angiotensin receptor antagonist](https://www.sec.gov/Archives/edgar/data/1438533/000143853326000063/tvtx-20260630.htm). It blocks two pathways that drive protein leakage and scarring in rare kidney diseases, and in preclinical work that reduced proteinuria and protected the kidney's filtering cells.

In the U.S., FILSPARI is approved to [slow kidney function decline in adults with IgA nephropathy and to reduce proteinuria in patients aged 8 and older with focal segmental glomerulosclerosis (FSGS) without nephrotic syndrome](https://www.sec.gov/Archives/edgar/data/1438533/000143853326000062/a2026q2_8-kxex991.htm). In the phase 3 DUPLEX trial, the subgroup without nephrotic syndrome had a [48% proteinuria reduction versus 27% on irbesartan at Week 108](https://www.sec.gov/Archives/edgar/data/1438533/000143853326000032/tvtx-20260413.htm). Behind it sit pegtibatinase, an enzyme replacement candidate for classical homocystinuria (HCU), and civorebrutinib, an oral BTK inhibitor licensed from Everest Medicines for immune-mediated kidney disease.

## The odds

FILSPARI's risk is now commercial, not regulatory. Both indications are approved, and the partner Chugai submitted a Japanese application in June, per the [second-quarter release](https://www.sec.gov/Archives/edgar/data/1438533/000143853326000062/a2026q2_8-kxex991.htm). The FSGS label is for lowering proteinuria, a narrower claim than the IgA nephropathy label on kidney function decline.

Pegtibatinase carries more science risk. The phase 3 HARMONY study enrolls about 70 patients, with topline data in the second half of 2027, and enrollment was [paused in September 2024](https://www.sec.gov/Archives/edgar/data/1438533/000143853326000063/tvtx-20260630.htm) after the drug substance profile fell short in manufacturing scale-up. The BIO, Informa and QLS study found that [57.8% of phase 3 programs advance to a filing](https://go.bio.org/rs/490-EHZ-999/images/ClinicalDevelopmentSuccessRates2011_2020.pdf). Civorebrutinib has phase 1/2 data only in primary membranous nephropathy, with FSGS and minimal change disease still to be tested. Demand indicators are positive: Travere received [2,012 new patient start forms](https://www.sec.gov/Archives/edgar/data/1438533/000143853326000062/a2026q2_8-kxex991.htm) in the quarter, though the figure combines both diseases and is not a sales number.

## The money

FILSPARI U.S. net sales were [$141.1 million in the second quarter, up 96% from a year earlier](https://www.sec.gov/Archives/edgar/data/1438533/000143853326000062/a2026q2_8-kxex991.htm). The reported net loss of $34.8 million included a $40.0 million expense tied to repurchasing convertible notes, and the non-GAAP net loss was $9.0 million.

Cash, cash equivalents and marketable securities were $489.2 million at June 30. The $112.5 million upfront payment for civorebrutinib was made in July, after that date. Debt at quarter end was [$525.0 million of 0.50% notes due 2032 and $94.9 million of 2.25% notes due 2029](https://www.sec.gov/Archives/edgar/data/1438533/000143853326000063/tvtx-20260630.htm). That is up from $316.3 million of convertible principal at year-end, after Travere refinanced its 2029 notes in May. Travere says cash plus operating cash flow should fund operations beyond the next 12 months.

## The risks

FILSPARI carries a [boxed warning for liver injury and fetal harm](https://www.sec.gov/Archives/edgar/data/1438533/000143853326000062/a2026q2_8-kxex991.htm) and is sold through a restricted program, the FILSPARI REMS, that requires enrolled prescribers, patients and pharmacies. The 10-Q says third-party manufacturers are currently sole-source suppliers for FILSPARI and Thiola.

Leadership is changing. Chief executive Eric Dube leaves on December 1, and the [research chief](https://www.sec.gov/Archives/edgar/data/1438533/000143853326000048/tvtx-20260612.htm) and the [chief accounting officer](https://www.sec.gov/Archives/edgar/data/1438533/000143853326000054/tvtx-20260706.htm) are also retiring in early 2027. A single trial, HARMONY, will decide whether pegtibatinase is a product.

## Our read

Travere has a fast-growing kidney franchise and a second approved indication, and it has the cash to fund its new programs. The sales line is strong, though profit is thin and the company has added about $300 million of convertible debt this year. The next test is whether a new chief executive can keep the FSGS launch on track while the HCU trial and the BTK program move forward. Most of the value beyond FILSPARI rests on programs that have not reached phase 3 results. Three dates matter: the December 1 leadership change, the FILSPARI sales trend over the next two quarters, and the second-half 2027 HARMONY readout.
