# Scholar Rock gets its approval; the launch is next

Isembyld cleared the FDA 19 days early. A 2.2-point motor gain, a fracture signal and a net loss near $110 million a quarter frame what comes next.

## The technology

Scholar Rock makes antibodies that block myostatin, a protein that limits muscle growth. Its lead drug, apitegromab (Isembyld), binds the pro and latent forms of myostatin in skeletal muscle and stops them from activating. The FDA approved it on Sept. 11 for spinal muscular atrophy in people aged two and older who are already on an [SMN2-targeted treatment](https://www.sec.gov/Archives/edgar/data/1727196/000110465926107273/tm2625362d1_ex99-1.htm). The company says it is the first muscle-targeted therapy for SMA.

The pipeline extends the same idea. Apitegromab is in a phase 2 trial in facioscapulohumeral muscular dystrophy and a phase 2 trial in infants and toddlers with SMA. A subcutaneous formulation is in development, and SRK-439, a subcutaneous myostatin inhibitor, has a [phase 1 healthy-volunteer study](https://www.sec.gov/Archives/edgar/data/1727196/000119312526336715/srrk-ex99_1.htm) due to report in late 2026.

## The odds

Scholar Rock has cleared the biggest hurdle for its lead drug. BIO, Informa and QLS data for 2011 to 2020 show [90.6% of applications at the BLA stage are approved](https://go.bio.org/rs/490-EHZ-999/images/ClinicalDevelopmentSuccessRates2011_2020.pdf), and 93.6% in rare disease. The company reached approval after a complete response letter tied to a contract manufacturing site, not to efficacy.

The clinical bar for the follow-ons is higher. In FSHD, the FORGE trial plans to enroll about 60 patients and measure lean muscle volume at 12 months. Early-stage programs face base rates such as 52.0% for phase 1 to phase 2 and 28.9% for phase 2 to phase 3. SRK-439 is at the earliest stage.

## The money

Scholar Rock had [$492.1 million of cash and securities](https://www.sec.gov/Archives/edgar/data/1727196/000119312526336715/srrk-ex99_1.htm) on June 30. It reported a quarterly net loss of $109.9 million and says its cash funds operations into the second half of 2027. It owed $200.0 million under a Blue Owl term loan, with a [delayed-draw commitment of up to $150.0 million](https://www.sec.gov/Archives/edgar/data/1727196/000119312526336732/srrk-20260630.htm) available from FDA approval.

The company also raised money through its at-the-market stock program, with about [$257.7 million of net proceeds](https://www.sec.gov/Archives/edgar/data/1727196/000110465926091702/tm2622083d2_8k.htm) to August and a new $200 million program filed on Aug. 6. Further dilution is likely if launch revenue ramps slowly.

## The risks

The label is narrow: it covers patients already on SMN2 therapy. In SAPPHIRE, [fractures occurred in 9% of patients](https://www.sec.gov/Archives/edgar/data/1727196/000110465926107273/tm2625362d1_ex99-1.htm) on the 10 mg/kg dose versus 2% on placebo. The FDA also required a pregnancy study and asked for 15-day fracture alert reports, which the company disclosed after management misstated the position on its call.

Commercial execution is untested. Scholar Rock has no earlier marketed product, and it is launching into a disease with established treatments. Europe is delayed, since the EMA is waiting on the FDA's classification of Catalent Indiana, and the company said it would [give new CHMP timing](https://www.sec.gov/Archives/edgar/data/1727196/000119312526336715/srrk-ex99_1.htm) later.

## Our read

In short, Scholar Rock turned a manufacturing setback into an approval inside twelve months, and that is real evidence that the science and the company's response held up. The 2.2-point HFMSE gain is modest in absolute terms, and the story from here is about adoption.

The next signals are plain: launch metrics in the third-quarter report, whether the delayed-draw loan is used, EMA timing and the fracture reporting record. Until those arrive, outside investors are weighing a first launch against a business whose [second-quarter net loss was $109.9 million](https://www.sec.gov/Archives/edgar/data/1727196/000119312526336715/srrk-ex99_1.htm). The disclosed facts are limited: a label restricted to patients on SMN2 therapy, and a company that says it is [working with commercial and government payers](https://www.sec.gov/Archives/edgar/data/1727196/000110465926107273/tm2625362d1_ex99-1.htm) to establish access.
