# Sarepta's next chapter depends on siRNA data, not Elevidys

Duchenne sales pay the bills and the Arrowhead fees. A new CEO has seven months of readouts and an FDA ruling to show the platform bet was worth it.

## The technology

Sarepta has two working platforms and is betting on a third. The exon-skipping drugs Exondys 51, Vyondys 53 and Amondys 45 are antisense drugs designed to raise dystrophin levels in muscle, and ESSENCE showed [dystrophin rose on treatment](https://www.neurologylive.com/view/fda-accepts-sarepta-sndas-casimersen-golodirsen-dmd). They sell about [$230 million a quarter](https://www.biospace.com/press-releases/sarepta-therapeutics-announces-second-quarter-2026-financial-results-and-recent-corporate-developments). Elevidys is an AAV gene therapy that delivers micro-dystrophin. Since November 2025 it carries a [boxed warning for acute liver failure](https://www.biospace.com/press-releases/fda-approves-new-safety-warning-and-revised-indication-that-limits-use-for-elevidys-following-reports-of-fatal-liver-injury) and is limited to ambulatory patients aged 4 and older.

The new bet is siRNA. Sarepta licensed an αvβ6 integrin-targeted delivery system from Arrowhead Pharmaceuticals that carries siRNA into skeletal muscle. First single-dose data in FSHD1 (SRP-1001) and DM1 (SRP-1003) showed drug levels in muscle rising with dose, lower target levels and mostly mild to moderate side effects, [per RARE Daily](https://globalgenes.org/raredaily/sarepta-reports-encouraging-clinical-data-from-sirna-pipeline-for-fshd1-and-dm1/). A Huntington's program, SRP-1005, has [started dosing](https://www.biospace.com/press-releases/sarepta-therapeutics-announces-second-quarter-2026-financial-results-and-recent-corporate-developments) in a phase 1 study.

## The odds

The siRNA programs sit in phase 1/2. Published data put the chance that a phase 1 drug reaches approval at [7.9% overall and 17.0% in rare disease](https://go.bio.org/rs/490-EHZ-999/images/ClinicalDevelopmentSuccessRates2011_2020.pdf). Early target knockdown in muscle helps, but single-dose biomarker data say little about function. The repeat-dose cohorts are the first real test, and Sarepta has not yet said which functional measures it will use to judge them.

The near-term regulatory test is ESSENCE. That study [missed its primary endpoint](https://www.neurologylive.com/view/fda-accepts-sarepta-sndas-casimersen-golodirsen-dmd) of 4-step ascend velocity, with a difference of 0.06 steps per second (P = .309), while dystrophin rose. The FDA will rule on traditional approval for Vyondys 53 and Amondys 45 by [Feb. 28, 2027](https://www.businesswire.com/news/home/20260630779541/en/Sarepta-Announces-FDA-Acceptance-of-sNDAs-for-AMONDYS-45-and-VYONDYS-53). The 90.6% base rate for filings reaching approval assumes positive confirmatory data, so it is a poor guide here.

## The money

Sarepta had [$945.0 million](https://www.biospace.com/press-releases/sarepta-therapeutics-announces-second-quarter-2026-financial-results-and-recent-corporate-developments) in cash, equivalents, restricted cash and investments on June 30, 2026, and its 10-Q lists [$847.6 million](https://www.sec.gov/Archives/edgar/data/0000873303/000119312526335003/srpt-20260630.htm) in convertible notes due in 2027 and 2030. The company says resources cover the operating plan for at least 12 months. Product revenue guidance for 2026 is $1.2 billion to $1.3 billion, and combined non-GAAP R&D and SG&A guidance is $800 million to $850 million.

The Arrowhead deal is the large fixed cost. Sarepta owes $50 million a year in license fees over five years and up to $10.3 billion in milestones, and it paid a $200 million DM1 milestone in the first half of 2026, [per the 10-Q](https://www.sec.gov/Archives/edgar/data/0000873303/000119312526335003/srpt-20260630.htm).

## The risks

Elevidys safety is the first risk. A new liver or cardiac event could shrink the label again, and a path back to non-ambulatory patients depends on sirolimus data due in the [first quarter of 2027](https://www.biospace.com/press-releases/sarepta-therapeutics-announces-second-quarter-2026-financial-results-and-recent-corporate-developments). The second is the exon-skipping franchise: an adverse FDA ruling would hit the drugs that now earn more than twice what Elevidys does, while Dyne Therapeutics' exon 51 candidate has a [Jan. 21, 2027](https://www.fiercepharma.com/pharma/sarepta-new-ceo-michael-severino-faces-critical-tests-prove-growth) decision date. The third is siRNA execution, where multiple-dose data have not yet been shown.

## Our read

Sarepta is a cash-generating rare disease company that has lost its growth story and is trying to buy a new one. The current business can carry the debt and the Arrowhead payments for now. What changes the picture is whether the siRNA platform shows a functional or strong biomarker effect in DM1 and FSHD with repeat dosing. New CEO Michael Severino, who [started July 28](https://www.biospace.com/press-releases/sarepta-therapeutics-appoints-michael-severino-m-d-chief-executive-officer), will be judged largely on those readouts and on the February FDA decision, both inside his first seven months. If the siRNA data disappoint, the company is left with a shrinking gene therapy and an exon-skipping franchise whose label depends on how the FDA reads a failed study.
