# Rhythm has an HO launch under way. The pipeline beyond it is unproven

IMCIVREE sales rose 19% in the second quarter on the acquired hypothalamic obesity launch. Genetic obesity expansion has failed once, and Prader-Willi data are early.

## The technology

Rhythm sells one drug, IMCIVREE (setmelanotide), a daily injection that switches on the melanocortin-4 receptor, or MC4R. The company says the MC4R pathway [controls hunger, energy balance and body weight](https://www.sec.gov/Archives/edgar/data/1649904/000162828026019995/ex991hopdufapressrelease.htm). Injury to the hypothalamus, most often after a brain tumor or its treatment, can reduce the signal that normally activates that pathway. Setmelanotide stands in for the missing signal.

The label covers acquired hypothalamic obesity (HO) in patients aged 4 and older, plus obesity caused by Bardet-Biedl syndrome (BBS) or by POMC, PCSK1 or LEPR deficiency, as listed in the [second-quarter release](https://www.sec.gov/Archives/edgar/data/1649904/000162828026052170/q22026earningspr_draft.htm). The HO approval rests on the phase 3 TRANSCEND trial, published in the [New England Journal of Medicine](https://pubmed.ncbi.nlm.nih.gov/42418774/), where setmelanotide cut BMI by 16.5% at 52 weeks against a 3.3% rise on placebo. Behind it sit bivamelagon, an oral MC4R agonist, and RM-718, a [weekly injection designed to spare the related MC1R receptor](https://www.sec.gov/Archives/edgar/data/1649904/000162828026052170/rytmq22026rm-718.htm), which the company says should reduce hyperpigmentation.

## The odds

Approval risk for HO is largely behind the company. The FDA approved the indication on [March 19](https://www.sec.gov/Archives/edgar/data/1649904/000162828026019995/ex991hopdufapressrelease.htm), and the [10-Q](https://www.sec.gov/Archives/edgar/data/1649904/000162828026052533/rytm-20260630.htm) records European Commission approval on April 30. A Japanese decision is expected by year-end, per the [Q2 release](https://www.sec.gov/Archives/edgar/data/1649904/000162828026052170/q22026earningspr_draft.htm).

Expansion beyond HO is less certain. The phase 3 EMANATE trial [missed its primary endpoint in all four genetic substudies](https://www.sec.gov/Archives/edgar/data/1649904/000162828026018211/ex991rytm20260616emanatepr.htm), a result the company has tried to recover with post hoc analyses. Its Prader-Willi syndrome (PWS) phase 2 showed a [mean BMI reduction of 3.06% at six months](https://www.sec.gov/Archives/edgar/data/1649904/000162828026043170/ex991rytmprpwsatendo2026.htm) in an open-label design with 17 patients, far below the HO result. Early programs also face long odds: the BIO, Informa and QLS study found that [7.9% of drugs entering phase 1 reach approval](https://go.bio.org/rs/490-EHZ-999/images/ClinicalDevelopmentSuccessRates2011_2020.pdf). Bivamelagon's phase 3 start is guided for year-end, and RM-718 has data from seven patients.

## The money

Rhythm reported second-quarter IMCIVREE revenue of [$71.3 million, up 19% from the first quarter](https://www.sec.gov/Archives/edgar/data/1649904/000162828026052170/q22026earningspr_draft.htm), and a net loss attributable to common stockholders of $50.4 million. Cash, cash equivalents and short-term investments were $330.9 million at June 30, down from $388.9 million at year-end. The company says that covers operations for at least 24 months.

Rhythm also owes revenue to a financing partner. Under its [royalty agreement with HealthCare Royalty](https://www.sec.gov/Archives/edgar/data/1649904/000162828026052533/rytm-20260630.htm), it pays 11.5% of annual net revenue up to $125 million, 7.5% between $125 million and $300 million, and 2.5% above that. It had paid $51.5 million cumulatively by June 30, and minimum-payment tests apply in March 2027 and March 2029.

## The risks

Concentration is the first risk. All revenue comes from IMCIVREE, and the 10-Q says about [72% of second-quarter revenue came from a single U.S. customer](https://www.sec.gov/Archives/edgar/data/1649904/000162828026052533/rytm-20260630.htm). The company also relies on third-party manufacturers and genetic testing providers.

Safety and access are the second. The label carries warnings for [depression and suicidal ideation, sexual adverse reactions and skin hyperpigmentation](https://www.sec.gov/Archives/edgar/data/1649904/000162828026052170/q22026earningspr_draft.htm). Revenue outside the U.S. carries pricing mechanisms of its own: Rhythm booked a $3.8 million retrospective charge in France in the second quarter.

## Our read

Rhythm has turned a rare-disease approval into a launch that is growing quickly, and it has cash for two years. The HO label is real and the early start-form count is encouraging, though it measures interest, not paid therapy. The company still loses about $50 million a quarter, and the push into genetic obesities has failed once while the Prader-Willi data are modest. The next-generation drugs matter because they decide whether this stays a one-product company.
