# Moderna's cancer bet pays off in phase 3; now it has to make and market it

Intismeran met its melanoma endpoint and a flu vaccine won approval. Revenue is still small, and full data come at ESMO.

## The technology

Moderna makes medicines from messenger RNA, which instructs cells to produce a target protein. Its approved products are vaccines: Spikevax and mNEXSPIKE for COVID-19, mRESVIA for RSV, mCOMBRIAX for flu plus COVID in Europe, and mFLUSIVA for flu, which the FDA approved for people 50 and older on [August 5, 2026](https://www.fda.gov/vaccines-blood-biologics/vaccines/mflusiva). In the flu trial, mFLUSIVA beat a standard-dose shot with 26.6% relative efficacy, per the [label](https://www.fda.gov/media/194120/download?attachment).

The larger bet is cancer. Intismeran autogene is a personalized mRNA therapy built from each patient's tumor mutations and given with Merck's Keytruda. The phase 3 INTerpath-001 trial in resected melanoma met its main goal of recurrence-free survival, the companies said on [August 19](https://www.merck.com/news/merck-and-moderna-announce-phase-3-interpath-001-trial-of-intismeran-autogene-plus-keytruda-met-endpoints-of-recurrence-free-survival-rfs-and-distant-metastasis-free-survival-dmfs-in-patient/).

## The odds

Intismeran has cleared phase 3 in melanoma. In the [BIO/Informa/QLS 2011 to 2020 study](https://go.bio.org/rs/490-EHZ-999/images/ClinicalDevelopmentSuccessRates2011_2020.pdf), 90.6% of drugs that reached an NDA or BLA were approved. The odds of some approval look good; the size of the label depends on hazard ratios that have not been released yet.

The rest of the pipeline is mixed. Intismeran is in [nine phase 2 and 3 trials](https://www.merck.com/wp-content/uploads/sites/124/2026/08/Merck-Moderna_INTerpath_Clinical-Program-Backgrounder.pdf), including three phase 3 trials in lung cancer. The propionic acidemia drug mRNA-3927 is in a registrational study, and rare disease programs reaching phase 3 move to filing 60.4% of the time in the BIO data. The norovirus vaccine missed an interim efficacy bar and needs more patients, per the [Q2 report](https://www.sec.gov/Archives/edgar/data/1682852/000168285226000147/exhibit9912026q2pressrelea.htm).

## The money

Moderna held $6.9 billion in cash and investments on June 30, 2026, then paid $950 million in July to settle litigation with Arbutus and Genevant, according to its [10-Q](https://www.sec.gov/Archives/edgar/data/1682852/000168285226000150/mrna-20260630.htm). In late August it priced [$2.6 billion of zero-coupon convertible notes](https://www.sec.gov/Archives/edgar/data/1682852/000119312526378505/d108896dex992.htm) due 2032, with a conversion price of about $210.58.

Revenue is small and seasonal: $145 million in the second quarter, against a $782 million net loss. The company guides to up to 10% revenue growth for 2026 and R&D of about $2.9 billion. It also has a $1.5 billion secured term loan facility, of which $600 million was funded.

## The risks

COVID vaccine demand keeps falling outside the fall season, and the company expects about 55% of second half revenue in the third quarter, per the [Q2 report](https://www.sec.gov/Archives/edgar/data/1682852/000168285226000147/exhibit9912026q2pressrelea.htm). mFLUSIVA's efficacy trial used a standard-dose comparator; its only high-dose comparison is an immune response study, per the [label](https://www.fda.gov/media/194120/download?attachment).

For intismeran, overall survival is immature and every dose is made for one patient, which makes manufacturing the main launch risk. Moderna created a chief operating officer role and rehired Juan Andres, its former manufacturing head, on [September 30](https://www.sec.gov/Archives/edgar/data/1682852/000119312526408289/d18337dex991.htm), while its quality and technical operations chief retired. The convertible notes could add up to about 14 million shares on conversion, per the [8-K](https://www.sec.gov/Archives/edgar/data/1682852/000119312526378505/d108896d8k.htm).

## Our read

Moderna has shifted from a COVID company with shrinking sales to a company with a credible cancer product. The INTerpath-001 result is the strongest proof yet that mRNA can work in oncology, and the convertible sale gives Moderna money to launch it. It develops the drug jointly with Merck, which supplies Keytruda, the partner drug in INTerpath-001 and in most trials listed in the [program backgrounder](https://www.merck.com/wp-content/uploads/sites/124/2026/08/Merck-Moderna_INTerpath_Clinical-Program-Backgrounder.pdf). But the full data are not out, the vaccine business still loses money, and Moderna has not yet shown it can make personalized doses at commercial scale. ESMO and the November Analyst Day will show how big the melanoma benefit is and what Moderna plans to spend.
