# Madrigal's MASH lead rides on a 2027 cirrhosis readout

Rezdiffra sales are up 71% and cash is steady. The company is buying add-on drugs while it waits for the trial that could open a sicker patient group.

## The technology

Madrigal sells one drug. Rezdiffra (resmetirom) is a once-daily pill that activates the thyroid hormone receptor beta in the liver. It was the first drug approved by both the FDA and the European Commission for [MASH with moderate to advanced fibrosis](https://madrigalpharma.gcs-web.com/node/17666/pdf), stages F2 to F3. In the phase 3 MAESTRO-NASH trial, the drug's effect held across common genetic risk groups, according to a [Journal of Hepatology analysis](https://finviz.com/news/399186/madrigal-announces-publication-in-the-journal-of-hepatology-demonstrating-consistent-rezdiffra-efficacy-across-common-mash-genetic-risk-variants) published in October.

The pipeline is being built around Rezdiffra through deals. Madrigal licensed [ARO-PNPLA3 from Arrowhead](https://ir.madrigalpharma.com/news-releases/news-release-details/madrigal-adds-clinical-stage-sirna-asset-targeting-pnpla3-its), a phase 1 siRNA drug, and says it now has seven siRNA programs. Its oral GLP-1 drug MGL-2086 [began phase 1 in June](https://www.sec.gov/Archives/edgar/data/0001157601/000162828026050775/exhibit991-63026earningspr.htm), and a study pairing Rezdiffra with Pfizer's DGAT2 inhibitor ervogastat is due to start in the [fourth quarter of 2026](https://ir.madrigalpharma.com/news-releases/news-release-details/madrigal-pharmaceuticals-reports-first-quarter-2026-financial).

## The odds

The largest open question is cirrhosis. Madrigal is running a phase 3 outcomes trial in compensated MASH cirrhosis (F4c), which management expects to [read out in 2027](https://www.marketbeat.com/instant-alerts/madrigal-pharmaceuticals-q2-earnings-call-highlights-2026-07-30/). No drug is approved for this group. Across the industry, phase 3 programs move to a filing about [57.8% of the time](https://go.bio.org/rs/490-EHZ-999/images/ClinicalDevelopmentSuccessRates2011_2020.pdf) and filings reach approval 90.6% of the time. Open-label data shown at EASL, where the share of patients at high risk of portal hypertension fell from [75% to 54.5% over two years](https://ir.madrigalpharma.com/news-releases/news-release-details/madrigal-presents-data-demonstrating-rezdiffra-reduced-markers), are encouraging but have no control group.

The add-on programs face normal early odds. Phase 1 drugs reach approval about [7.9% of the time](https://go.bio.org/rs/490-EHZ-999/images/ClinicalDevelopmentSuccessRates2011_2020.pdf).

## The money

Second-quarter Rezdiffra sales were [$364.3 million, up 71%](https://www.sec.gov/Archives/edgar/data/0001157601/000162828026050775/exhibit991-63026earningspr.htm), and more than 49,000 patients were on therapy at June 30. The company still lost $57.9 million in the quarter, as operating expenses reached $420.6 million. Cash, equivalents and securities were $838.9 million at June 30, slightly up from [$817.9 million at March 31](https://ir.madrigalpharma.com/news-releases/news-release-details/madrigal-pharmaceuticals-reports-first-quarter-2026-financial) and down from $988.6 million at the end of 2025.

Management said trailing 12-month sales were [nearly $1.3 billion](https://www.marketbeat.com/instant-alerts/madrigal-pharmaceuticals-q2-earnings-call-highlights-2026-07-30/) and estimated that only about 10% of patients are diagnosed and about 10% of those are treated.

## The risks

Concentration is the first risk. Every dollar of revenue comes from one drug in one main market. Europe has started slowly, and CEO Bill Sibold said [reimbursement remains difficult](https://www.marketbeat.com/instant-alerts/madrigal-pharmaceuticals-q2-earnings-call-highlights-2026-07-30/) amid most-favored-nation pricing uncertainty.

The second is the cirrhosis trial. A miss would cap Rezdiffra at F2 to F3 patients and weaken the new [F4c method patent](https://ir.madrigalpharma.com/news-releases/news-release-details/madrigal-pharmaceuticals-announces-three-new-resmetirom-patents), which only has value with a cirrhosis label.

The third is the cost of growth. Selling and admin spending was [$289.4 million in the quarter](https://www.sec.gov/Archives/edgar/data/0001157601/000162828026050775/exhibit991-63026earningspr.htm), and the deal pace adds R&D costs. Madrigal's patent news covers methods of use; the release on those patents did not give compound patent expiry dates.

## Our read

Madrigal has the leading MASH drug and a market that is still mostly untapped. Sales growth is strong and cash held steady in the second quarter, which suggests the business is near the point of funding itself. The company is spending to defend the franchise with combinations before rivals arrive with their own.

The [2027 cirrhosis readout](https://www.marketbeat.com/instant-alerts/madrigal-pharmaceuticals-q2-earnings-call-highlights-2026-07-30/) is the event that will define the next phase. A win would open a sicker patient group with no approved option and support the new patent. A miss would leave Madrigal as a single-indication, single-product company leaning on early-stage deals to grow. In the meantime, investors can track progress through quarterly patient counts, the pace of the European rollout and the first clinical data from the combination programs, none of which has a firm date yet.
