# Incyte has the cash and two years to replace Jakafi

Second-quarter sales rose 38%, but a one-time CMS item did most of the work. Incyte is buying and partnering to fill the gap when Jakafi loses exclusivity.

## The technology

Incyte sells JAK inhibitors, drugs that block signaling enzymes involved in blood cancers and immune disease. Its anchor is Jakafi (ruxolitinib), which treats myelofibrosis, polycythemia vera and graft-versus-host disease. Opzelura is a cream version approved for skin conditions including vitiligo. Povorcitinib is a newer oral JAK inhibitor, in a [filed NDA for hidradenitis suppurativa](https://investor.incyte.com/node/26526/pdf) and phase 3 trials in vitiligo and prurigo nodularis.

Beyond JAKs, Incyte is building a hematology and oncology pipeline: an antibody against mutant calreticulin (INCA033989) in phase 3 for essential thrombocythemia, a KRAS G12D inhibitor in phase 3 for first-line pancreatic cancer, and latarcibart, acquired from Vega, for von Willebrand disease. It has also signed AI partnerships with [Genesis Molecular AI and Edison Scientific](https://www.genengnews.com/topics/artificial-intelligence/small-molecules-to-big-partnership-incyte-genesis-expand-ai-collaboration-to-1b/).

## The odds

Several decisions are near. Povorcitinib in hidradenitis suppurativa has an accepted NDA and Incyte expects a U.S. approval in the first quarter of 2027. The BIO/Informa/QLS base rates show that [90.6% of filed applications reach approval](https://go.bio.org/rs/490-EHZ-999/images/ClinicalDevelopmentSuccessRates2011_2020.pdf). The company also expects U.S. approval of Monjuvi in newly diagnosed lymphoma in the first quarter of 2027, after the frontMIND phase 3 met its [primary endpoint](https://www.sec.gov/Archives/edgar/data/0000879169/000087916926000053/incy-q22026xexx991.htm).

The harder bets are earlier. The mutCALR antibody started phase 3 in mid-2026, so it still faces the BIO/Informa/QLS base rate of [57.8% of phase 3 programs reaching a filing](https://go.bio.org/rs/490-EHZ-999/images/ClinicalDevelopmentSuccessRates2011_2020.pdf). Vitiligo data for povorcitinib were positive but trail Rinvoq's placebo-adjusted results, per [Fierce Biotech](https://www.fiercebiotech.com/biotech/incytes-jak-inhibitor-scores-phase-3-vitiligo-wins-cant-quite-shake-rinvoq).

## The money

Incyte is profitable. Second-quarter total revenue was [$1.67 billion, up 38%](https://www.sec.gov/Archives/edgar/data/0000879169/000087916926000053/incy-q22026xexx991.htm), with net income of $585.6 million on a GAAP basis. Without a one-time $246 million Opzelura benefit from a CMS settlement, net sales grew 17%. Cash and securities were $4.5 billion at June 30.

The company raised 2026 net sales guidance to $5,130 million to $5,260 million. It will take a charge of about $1.27 billion in the third quarter for the Vega acquisition, which cost [$1.25 billion upfront](https://www.biopharmadive.com/news/incyte-vega-acquisition-star-biotech-deal-blood-disorder-vwd/822238/) plus up to $750 million in sales milestones. GAAP spending guidance rose accordingly.

## The risks

The first risk is Jakafi. It made [71% of Incyte's nearly $4.4 billion in net product revenue](https://www.biopharmadive.com/news/incyte-vega-acquisition-star-biotech-deal-blood-disorder-vwd/822238/) last year and loses exclusivity between late 2028 and early 2029. Jakafi XR, approved in May, is the company's first mitigation, but it targets only [3% to 5% of patients](https://www.fiercepharma.com/pharma/boosted-cms-settlement-incyte-expects-opzelura-cross-1b-2026).

The second is policy. Opzelura's sales depend on a CMS settlement, and CEO Bill Meury says the pricing and reimbursement environment is dynamic. The third is competition: AbbVie's Rinvoq is filed in vitiligo with stronger placebo-adjusted results, and Opzelura's underlying growth was 24% in the quarter, less than the headline. The fourth is execution across a pipeline of ten phase 3 studies.

## Our read

Incyte is a profitable company trying to turn one dominant drug into a diversified one before the patents run out. The most important numbers in the quarter were the underlying ones: 17% net sales growth, Jakafi up 7%, Opzelura up 24% before the settlement benefit.

The strategy has been to buy and partner, with Vega, Genesis and Edison all announced within a few weeks, and to fund it from Jakafi cash. That works if the pipeline produces replacement revenue by 2029. The checkpoints are the first-quarter 2027 decisions on povorcitinib and Monjuvi, the fourth-quarter data in hidradenitis suppurativa and prurigo nodularis, and the October ESMO presentations. Each can move the story, and none by itself replaces Jakafi's $3.2 billion.
