# EyePoint's Duravyu now rests on diabetic macular edema

Two wet AMD phase 3 misses leave the insert's future on two DME trials that read out in late 2027, with about $118 million in the bank and half the staff gone.

## The technology

Duravyu is vorolanib, a tyrosine kinase inhibitor that blocks VEGF receptors, packed into a bioerodible insert injected into the eye in the doctor's office. The idea is to replace shots every eight weeks with a dose every six months. In preclinical work shown at ARVO 2026, EyePoint said vorolanib also [inhibits IL-6 signaling](https://app.edgar.tools/filing/1314102/0001193125-26-207537/eypt-ex99_1.htm) and acts on JAK1.

The durability part works. In LUGANO, Duravyu patients needed [3.1 injections after loading against 5.3](https://investors.eyepoint.bio/static-files/7e8b3b5e-2f94-4f34-8d97-586752ec3cb2) for aflibercept. In LUCIA, the cut was [38%](https://www.ophthalmologytimes.com/view/duravyu-misses-noninferiority-end-point-in-phase-3-lucia-wet-amd-trial). The vision part did not. LUCIA's Duravyu eyes gained 2.5 letters against 6.1 for aflibercept, outside the 4.5-letter non-inferiority margin. LUGANO also missed in its full dataset.

## The odds

The drug now rests on two DME trials, COMO and CAPRI, with topline data in the [fourth quarter of 2027](https://www.sec.gov/Archives/edgar/data/0001314102/000119312526424644/eypt-ex99_1.htm). Each tests Duravyu 2.7 mg against aflibercept on a vision non-inferiority endpoint, the same design that failed twice in wet AMD.

The general base rates from the [BIO, Informa and QLS 2011 to 2020 study](https://go.bio.org/rs/490-EHZ-999/images/ClinicalDevelopmentSuccessRates2011_2020.pdf) give a phase 3 program a 57.8% chance of reaching a filing and a 52.4% chance of approval. Those numbers assume nothing about prior results. Here, the same molecule and dose have already missed a vision non-inferiority test against the same comparator in two phase 3 trials. In our view, that puts the DME odds below the base rate, though DME is a different disease and the phase 2 VERONA trial showed a [two-thirds cut in treatment burden](https://www.biopharmawatch.com/news/EYPT/eyepoint-completes-enrollment-phase-3-trials-duravyu-dme).

## The money

EyePoint reported about [$118 million in cash and investments](https://www.ophthalmologytimes.com/view/duravyu-misses-noninferiority-end-point-in-phase-3-lucia-wet-amd-trial) at September 30, down from $180 million at June 30 and [$306 million](https://app.edgar.tools/filing/1314102/0001193125-26-207537/eypt-ex99_1.htm) at the end of 2025. Its second-quarter net loss was [$94.5 million](https://www.biopharmawatch.com/news/EYPT/eyepoint-reports-q2-2026-results-and-developments).

The October plan cuts about [53% of the workforce](https://finviz.com/news/403991/eyepoint-announces-53-workforce-reduction-following-phase-3-duravyu-trial-results) for about $30 million in annual savings, with about $9 million in one-time charges. Most cuts land in the fourth quarter of 2026. In August the company had guided that cash would last into the fourth quarter of 2027, but that guidance came before the restructuring, and the sources reviewed here give no updated runway. Revenue is minimal: total net revenue was [$0.5 million](https://www.biopharmawatch.com/news/EYPT/eyepoint-reports-q2-2026-results-and-developments) in the second quarter.

## The risks

The first risk is a third phase 3 miss. DME trials enroll a different population, but the [vision deficit](https://www.ophthalmologytimes.com/view/duravyu-misses-noninferiority-end-point-in-phase-3-lucia-wet-amd-trial) in LUCIA pointed the same way as LUGANO.

The second is funding. With $118 million and a readout about five quarters away, EyePoint may need to raise money or find a partner before it has new data, which would likely mean dilution. Shares fell [85% on October 19](https://finviz.com/news/403991/eyepoint-announces-53-workforce-reduction-following-phase-3-duravyu-trial-results), according to Finviz market data.

The third is safety optics, which matter in a market where retina doctors have long experience with aflibercept. Duravyu had more [conjunctival hemorrhage, floaters and retinal hemorrhage](https://www.ophthalmologytimes.com/view/duravyu-misses-noninferiority-end-point-in-phase-3-lucia-wet-amd-trial) than aflibercept in LUCIA.

## Our read

EyePoint built a solid case for durability and could not show that durability came without a vision cost. Two trials with [more than 900 patients](https://www.biopharmawatch.com/news/EYPT/eyepoint-announces-third-consecutive-positive-dsmc-recommendation-for-phase-3-wet-amd-tria-eypt-20260514) gave the same answer, which leaves little room for the outlier argument the company made in August.

What remains is a fully enrolled DME program, a smaller company and a long wait. The DME readout is a true binary event, and the October cuts look sized to reach it. Until late 2027, the balance sheet is likely to drive the news more than the science, starting with whether EyePoint gives [updated runway guidance](https://retinalphysician.com/news/2026/eyepoint-to-cut-workforce-after-lucia-trial-failure/) after the restructuring.
