# CSL resets around plasma as Vifor shrinks

A $2.6 billion loss, a guidance cut and a class action mark a hard year. A new CEO arrives in February to a business that still throws off cash.

## The technology

CSL is mostly a plasma company. Its CSL Behring unit collects human plasma and turns it into immunoglobulin, albumin and clotting-factor products, and it had [$11.4 billion of FY26 revenue](https://announcements.asx.com.au/asxpdf/20260818/pdf/072vps0nphgdnj.pdf) out of a group total of $15.8 billion. CSL Seqirus makes flu vaccines, including Fluad, which sold $968 million. CSL Vifor sells iron and kidney drugs.

Newer products are small but growing. Hemgenix, a gene therapy for hemophilia B, grew 25%, and Andembry, for hereditary angioedema, sold $240 million in its first full year. CSL is also trying to raise yield with Horizon 2, a patented process for getting more immunoglobulin from the same plasma, but it now needs [clinical trials starting in mid-2027](https://announcements.asx.com.au/asxpdf/20260729/pdf/07257sm52ttd3q.pdf) before regulators will sign off.

## The odds

Most of CSL's revenue comes from approved products, so the main uncertainty is commercial, not clinical. The pipeline is thin. CSL's chair wrote in the 2026 meeting notice that ["our research and development efforts didn't deliver."](https://announcements.asx.com.au/asxpdf/20260917/pdf/0745r5xv26f9y2.pdf)

The newest bets are early or mid-stage. Lixudebart, licensed from Alentis in October for [$355 million upfront](https://newsroom.csl.com/2026-10-04-CSL-and-Alentis-announce-global-partnership-to-develop-and-commercialise-lixudebart-for-rare-kidney-and-liver-diseases), is in phase 2, where BIO, Informa and QLS data put the odds of reaching phase 3 at [28.9%](https://go.bio.org/rs/490-EHZ-999/images/ClinicalDevelopmentSuccessRates2011_2020.pdf). Clazakizumab is in phase 3 for cardiovascular events in end-stage kidney disease, and VMX-C001 has [started phase 3](https://announcements.asx.com.au/asxpdf/20260818/pdf/072vq7zfyd3zhp.pdf). For phase 3 programs, the same data show 57.8% reach a filing.

## The money

CSL ended FY26 with [cash of $1,513 million](https://announcements.asx.com.au/asxpdf/20260818/pdf/072vppn8s55bwq.pdf) and leverage of [1.8 times net debt to EBITDA](https://announcements.asx.com.au/asxpdf/20260818/pdf/072vps0nphgdnj.pdf). Operating cash flow was $3.5 billion. It maintained its dividend and announced an A$1.1 billion buyback. In October it [priced $1.5 billion of bonds](https://announcements.asx.com.au/asxpdf/20261007/pdf/0751343ljhscjd.pdf) at 5.948% and 6.201% to refinance debt.

The income statement looks worse than the cash. A reported net loss of $2.6 billion came from $7.1 billion of pre-tax impairments and $799 million of restructuring costs. FY27 guidance calls for flat revenue and about 5% underlying NPAT growth.

## The risks

Vifor is shrinking. CSL expects its revenue to [fall about 25% in FY27](https://announcements.asx.com.au/asxpdf/20260818/pdf/072vps0nphgdnj.pdf) on generic iron, the end of Velphoro's U.S. reimbursement period and the loss of Tavneos, which the EU revoked after the EMA found its main trial [breached good clinical practice](https://www.ema.europa.eu/en/documents/referral/tavneos-article-20-procedure-ema-recommends-revoking-marketing-authorisation-tavneos_en.pdf).

U.S. policy is another exposure. CSL agreed in August to give Medicaid prices [comparable to other developed countries](https://newsroom.csl.com/2026-08-31-CSL-Enters-Agreements-with-the-U-S-Government-to-Lower-Drug-Prices-and-Support-Investment-in-U-S-Manufacturing) and to price new launches the same way for all U.S. payers, though it expects no material FY27 impact. A shareholder class action filed in October covers [statements about FY26 performance](https://announcements.asx.com.au/asxpdf/20261022/pdf/075mkm99xb7rjc.pdf). Leadership is in transition until Deborah Waterhouse starts on Feb. 1, 2027.

## Our read

CSL is a cash-generating plasma business that spent heavily on acquisitions and research that did not pay back, and it is now writing those bets down. The FY27 guide is modest, which makes it easier to meet. The plasma side looks steady, and [immunoglobulin demand is still growing](https://announcements.asx.com.au/asxpdf/20260511/pdf/06zfvkkjj67msw.pdf), according to the company.

What would change the picture is margin. If collection costs fall with new Haemonetics machines and Horizon 2 clears its trials, earnings can grow faster than revenue. If Vifor keeps shrinking and new deals like lixudebart stall, CSL stays a slow-growth company. We would watch Waterhouse's first strategy statement closely, since she takes over on [Feb. 1, 2027](https://newsroom.csl.com/2026-10-21-CSL-appoints-Deborah-Waterhouse-as-CEO-Managing-Director) with a board that has already said what it wants: renewed pipeline, simpler operations and continued commercial momentum.
