# Corcept's cancer launch is working. December decides the rest

Lifyorli helped lift Corcept's 2026 forecast to at least $1.1 billion. An FDA ruling on relacorilant in Cushing's syndrome, due Dec. 17, will decide how well it can defend against Korlym generics.

## The technology

Corcept makes drugs that block the effects of cortisol. Its first product, Korlym (mifepristone), has been sold since 2012 for hypercortisolism, also called Cushing's syndrome. Its newer molecules, led by relacorilant, are [selective glucocorticoid receptor antagonists](https://www.sec.gov/Archives/edgar/data/1088856/000162828026050607/cort072926ex991pressrelease.htm). Corcept says relacorilant avoids some serious side effects of approved Cushing's drugs, including [termination of pregnancy and hypokalemia](https://www.sec.gov/Archives/edgar/data/1088856/000119312526274258/d159941dex991.htm).

Relacorilant now has its first approval, as Lifyorli, with nab-paclitaxel for platinum-resistant ovarian cancer. In the phase 3 ROSELLA trial it cut the risk of death by 35%, with [median survival of 16.0 months versus 11.9 months](https://ir.corcept.com/news-releases/news-release-details/corcept-presents-new-data-asco-2026-lifyorlitm-overall-survival). Corcept calls that approval ["the first step in advancing glucocorticoid receptor antagonism as a treatment for many types of cancer"](https://www.sec.gov/Archives/edgar/data/1088856/000162828026050607/cort072926ex991pressrelease.htm) and is testing relacorilant in endometrial, cervical, pancreatic and other ovarian cancers.

## The odds

Corcept's near-term events are mostly regulatory, and one of them has already gone wrong once. The FDA decision on relacorilant in Cushing's syndrome is due by [December 17, 2026](https://www.sec.gov/Archives/edgar/data/1088856/000162828026050613/cort-20260630.htm). That application was rejected in December 2025 for insufficient evidence of efficacy and resubmitted with new analyses, not a new trial, so the outcome is uncertain. In Europe, the CHMP has [recommended approval of Lifyorli](https://ir.corcept.com/news-releases/news-release-details/corcept-announces-chmp-opinion-recommending-eu-marketing), with a Commission decision expected in the fourth quarter.

The FDA's December 2025 letter said the GRACE trial met its primary endpoint and that GRADIENT gave confirmatory evidence, yet asked for [more evidence of efficacy](https://www.sec.gov/Archives/edgar/data/1088856/000162828026050613/cort-20260630.htm). That is an unusual combination, and it makes the resubmission hard to handicap. Approval of a filed application is 90.6% likely historically across all indications, but that [BIO base rate](https://go.bio.org/rs/490-EHZ-999/images/ClinicalDevelopmentSuccessRates2011_2020.pdf) does not isolate resubmissions after a rejection.

The oncology expansion carries more risk. Most cancer trials report by the end of 2027, and phase 2 programs historically advance to phase 3 [28.9% of the time](https://go.bio.org/rs/490-EHZ-999/images/ClinicalDevelopmentSuccessRates2011_2020.pdf).

## The money

Corcept returned to profit in the second quarter and is growing, which is unusual for a company with a pipeline this broad. Second-quarter revenue was [$256.1 million](https://www.sec.gov/Archives/edgar/data/1088856/000162828026050607/cort072926ex991pressrelease.htm), up 32%, with net income of $43.0 million after a $31.8 million net loss in the first quarter. It raised 2026 guidance to $1.1 billion to $1.2 billion. Cash and investments were $544.6 million at June 30. Operating cash flow was only [$16.8 million in the first half](https://www.sec.gov/Archives/edgar/data/1088856/000162828026050613/cort-20260630.htm) as launch spending rose, down from $49.1 million. Operating expenses were [$214.8 million in the second quarter](https://www.sec.gov/Archives/edgar/data/1088856/000162828026050607/cort072926ex991pressrelease.htm), up from $167.8 million, because of the Lifyorli launch and investment in the Cushing's business. Corcept did not repurchase stock in the first half of 2026, after repurchasing $130.5 million in the same period of 2025, according to its cash flow statement. It gives no runway guidance.

## The risks

The main risk is Korlym's exposure to generics. Teva launched a [generic in January 2024](https://www.sec.gov/Archives/edgar/data/1088856/000162828026050613/cort-20260630.htm) and won on appeal in February 2026, and settlements allow Sun and Hikma to enter under certain conditions. Korlym and its authorized generic were still $208.6 million of second-quarter revenue, about 81% of the total, which is why the Cushing's decision matters so much.

Legal risk is the second. Teva and a group of insurers including Aetna and Humana have [sued Corcept on antitrust grounds](https://www.sec.gov/Archives/edgar/data/1088856/000162828026050613/cort-20260630.htm), and a former employee is pursuing a [whistleblower suit over Korlym marketing](https://www.sec.gov/Archives/edgar/data/1088856/000119312526254622/d103585d8k.htm) after the Justice Department declined to join it.

## Our read

Corcept has done what few small drugmakers manage: it turned one cortisol drug into a profitable business and then won a second approval in a new disease. Lifyorli's fast start and the raised guidance show the oncology bet is paying early. The December FDA decision is the next real test. An approval would give Corcept a patent-protected Cushing's drug as Korlym generics spread. A second rejection would leave the endocrinology business more exposed. Doctors' screening habits may also change: a [2026 ADA and EASD consensus report](https://ir.corcept.com/news-releases/news-release-details/2026-adaeasd-consensus-report-recommends-screening) now says screening for hypercortisolism should be considered in adults with difficult-to-control type 2 diabetes, which could enlarge the market whichever drug patients end up taking.
