# BioMarin bets on breadth while Voxzogo faces its first rival

Amicus added two growing drugs and $4 billion of debt. The next test is whether Voxzogo holds share and the pipeline delivers before 2028.

## The technology

BioMarin, founded in 1997, has [nine commercial therapies](https://www.biomarin.com/?p=12067), most of them enzyme replacement therapies. Its growth engine is [Voxzogo](https://www.biomarin.com/?p=12083), a daily injection that mimics C-type natriuretic peptide to boost bone growth in children with achondroplasia. It is approved in the U.S., Japan, Australia and Europe and has treated more than 5,000 children. The April purchase of [Amicus Therapeutics](https://www.biopharmawatch.com/news/BMRN/investors-media-traci-mccarty-marni-kottle-biomarin-pharmaceutical-inc-biomarin-pharmaceut-bmrn-20260427) added Galafold, an oral chaperone for Fabry disease, and Pombiliti plus Opfolda for Pompe disease.

The newer pipeline is thinner. BMN 333 is a long-acting CNP analog in a [phase 2/3 study](https://www.sec.gov/Archives/edgar/data/0001048477/000104847726000015/earningsrelease-6aug26ex991.htm) for achondroplasia. BMN 351 is in phase 1/2 for Duchenne muscular dystrophy. BMN 820, an [oral CCR2 inhibitor](https://www.nasdaq.com/articles/biomarin-pharmaceutical-q2-earnings-call-highlights) for the kidney disease FSGS, is in phase 3 with partner Dimerix. ALE1, from the [Alesta deal](https://www.biomarin.com/?p=11928), is a phase 1/2a pill for hypophosphatasia.

## The odds

The nearest regulatory events carry low scientific risk because they are label expansions of an approved drug. The hypochondroplasia trial [hit every growth endpoint](https://www.biomarin.com/?p=12083), and the BIO industry study puts the chance of approval at [93.6% for rare disease drugs](https://go.bio.org/rs/490-EHZ-999/images/ClinicalDevelopmentSuccessRates2011_2020.pdf) once an application is filed. The FDA decision on full approval in achondroplasia is due February 28, 2027.

Earlier assets face normal odds. From phase 1, rare disease drugs reach approval about [17.0% of the time](https://go.bio.org/rs/490-EHZ-999/images/ClinicalDevelopmentSuccessRates2011_2020.pdf), and that applies to ALE1 and BMN 351. BioMarin's recent record is a reminder: BMN 401 [raised its target biomarker in phase 3](https://www.biomarin.com/?p=11308) and still failed on the clinical endpoint.

## The money

Second-quarter revenue was [$990 million, up 20%](https://www.sec.gov/Archives/edgar/data/0001048477/000104847726000015/earningsrelease-6aug26ex991.htm). Guidance for 2026 calls for $3.875 billion to $3.925 billion in revenue and at least $1.0 billion from Voxzogo. The Amicus deal cost about $5.07 billion net of cash acquired. Cash fell to $874 million at June 30, against debt of about $4.19 billion. Management expects about $200 million a year in interest on the acquisition debt and targets gross leverage below 2.5 times by mid-2027.

The Alesta purchase will add about [$320 million of pre-tax charges](https://www.sec.gov/Archives/edgar/data/0001048477/000104847726000019/bmrn-20261006.htm) in 2026, cutting full-year EPS by about $1.55 compared with the August guidance, which did not include the deal.

## The risks

The largest commercial risk is competition for Voxzogo. Ascendis won U.S. accelerated approval for once-weekly [Yuviwel](https://pharmaphorum.com/news/ascendis-scores-fda-approval-weekly-dwarfism-drug) earlier in 2026. Management said about [10% of U.S. Voxzogo patients](https://www.nasdaq.com/articles/biomarin-pharmaceutical-q2-earnings-call-highlights) switched in the first six months, fewer than 100 children. About three quarters of Voxzogo revenue comes from outside the U.S., which limits the near-term exposure, but the U.S. is where the switching data come from.

The second risk is leverage. BioMarin took on about [$3.7 billion of new debt](https://www.sec.gov/Archives/edgar/data/0001048477/000104847726000006/earningsrelease-4may26ex991.htm) to pay for Amicus. That limits room for large deals until leverage falls, and it makes the Amicus cost savings, about [$220 million a year by 2028](https://www.sec.gov/Archives/edgar/data/0001048477/000104847726000015/earningsrelease-6aug26ex991.htm), something the company has to deliver.

The third is pipeline depth. After [BMN 401](https://www.biomarin.com/wp-content/uploads/2026/07/Update-on-BMN-401-Program.pdf), BioMarin has no internally discovered late-stage drug outside the CNP class.

## Our read

BioMarin is a profitable, growing rare disease company that has traded balance sheet strength for a broader product base. Voxzogo is [close to $1 billion a year](https://www.sec.gov/Archives/edgar/data/0001048477/000104847726000015/earningsrelease-6aug26ex991.htm), and the hypochondroplasia filing gives it a second indication that its main rival does not have. The Amicus drugs add growth that does not depend on achondroplasia.

What the company lacks is proof that its acquisition strategy produces pipeline wins. Two of its three recent deals are commercial or early stage, and the third ended in a phase 3 failure. The next year will test whether BioMarin can pay down debt and keep Voxzogo's share while it waits for [BMN 333 and BMN 820 data](https://www.sec.gov/Archives/edgar/data/0001048477/000104847726000015/earningsrelease-6aug26ex991.htm) in 2027 and 2028.
