# Axsome bets on Auvelity's elderly launch to turn profitable

A second Auvelity approval and a new FDA filing give Axsome momentum. Losses persist, and most of the pipeline is still in phase 3.

## The technology

Axsome sells three central nervous system drugs and is testing several more. Auvelity combines dextromethorphan, which acts on NMDA and sigma-1 receptors, with bupropion, which raises dextromethorphan levels in the blood. It is approved for major depressive disorder and, since April 30, for [agitation associated with Alzheimer's disease dementia](https://www.sec.gov/Archives/edgar/data/1579428/000119312526197857/axsm-ex99_1.htm). Sunosi (solriamfetol) treats excessive daytime sleepiness, and Symbravo treats migraine.

Most of the pipeline reuses known molecules in new diseases. Solriamfetol is in phase 3 for ADHD, binge eating disorder, shift work disorder and depression with sleepiness. AXS-05, the Auvelity combination, is in a [phase 3 smoking cessation trial](https://axsometherapeuticsinc.gcs-web.com/node/13246). AXS-12 (reboxetine) is filed for cataplexy in narcolepsy. Axsome also bought balipodect, a PDE10A inhibitor, from Takeda for [$10.4 million upfront](https://www.sec.gov/Archives/edgar/data/1579428/000119312526342360/axsm-20260630.htm).

## The odds

The near-term regulatory event is AXS-12, with an FDA [action date of May 1, 2027](https://www.sec.gov/Archives/edgar/data/1579428/000119312526304039/axsm-ex99_1.htm) and no advisory committee planned. BIO, Informa and QLS data for 2011 to 2020 show [90.6% of filed drugs are approved](https://go.bio.org/rs/490-EHZ-999/images/ClinicalDevelopmentSuccessRates2011_2020.pdf), and 93.6% in rare diseases.

The phase 3 programs carry more risk. The same data put the odds of a phase 3 program reaching a filing at 57.8%. Known safety helps, because these molecules are already approved for other uses, but each new indication must show benefit on its own. ENGAGE in binge eating disorder reads out in the [fourth quarter of 2026](https://www.sec.gov/Archives/edgar/data/1579428/000119312526341206/axsm-ex99_1.htm).

## The money

Axsome had [cash of $319.9 million at June 30, 2026](https://www.sec.gov/Archives/edgar/data/1579428/000119312526341206/axsm-ex99_1.htm), down slightly from $322.9 million at the end of 2025. It owed [$120.0 million in term loans and $70.0 million on a revolver](https://www.sec.gov/Archives/edgar/data/1579428/000119312526342360/axsm-20260630.htm) to Blackstone. The company says its cash is enough to fund operations into cash flow positivity under its current plan.

Revenue is growing fast. Second-quarter net product revenue was $218.4 million, up 46%, but the quarter still showed a $51.3 million net loss because selling, general and administrative costs reached $208.1 million.

## The risks

Axsome reported that payer coverage for Auvelity was about [89% of lives](https://www.sec.gov/Archives/edgar/data/1579428/000119312526341206/axsm-ex99_1.htm), which supports the launch, but the company still depends on Auvelity, which made up [$180.3 million of second-quarter sales](https://www.sec.gov/Archives/edgar/data/1579428/000119312526341206/axsm-ex99_1.htm). Its growth in Alzheimer's agitation will be compared with Otsuka's Rexulti, the [first drug approved for the condition](https://www.clinicaltrialsarena.com/analyst-comment/auvelity-first-non-antipsychotic-alzheimers-agitation/). Symbravo has been slow, with $2.3 million in sales and coverage for about 57% of lives.

Spending is the other pressure. Several phase 3 trials, a larger sales force and new launches all compete for the same cash. Sunosi's generic risk is low after a settlement that keeps copies out until at least [March 1, 2040](https://www.sec.gov/Archives/edgar/data/1579428/000119312526254708/axsm-ex99_1.htm).

## Our read

In plain terms, Axsome is a commercial company with a pipeline built around reformulating and extending known drugs. That lowers scientific risk compared with a company testing new targets, and it lets the sales force carry more products. The company has shown it can win approvals, with [two Auvelity indications](https://www.sec.gov/Archives/edgar/data/1579428/000119312526197857/axsm-ex99_1.htm) developed under Breakthrough Therapy designation and reviewed under Priority Review.

Management's own framing is that commercial momentum should [build over the rest of the year](https://www.sec.gov/Archives/edgar/data/1579428/000119312526341206/axsm-ex99_1.htm), especially for Auvelity in Alzheimer's agitation and depression. The next year is about whether Auvelity's elderly launch is large enough to turn the income statement positive. If it is, the many phase 3 readouts become upside. If growth slows, the cost of running those trials will weigh on a balance sheet that has $320 million of cash at June 30 against $190 million of debt, $70 million of which was repaid in July. Investors will also track whether the 2040 Sunosi settlement dates hold up after review by the [FTC and Justice Department](https://www.sec.gov/Archives/edgar/data/1579428/000119312526254708/axsm-ex99_1.htm), and whether AXS-12 gets through the FDA without an advisory committee. Each is a dated, checkable event, which makes the next twelve months easier to follow than most small-cap biotech stories.
