# Astellas is outrunning XTANDI's expected decline, for now

PADCEV keeps adding bladder cancer indications and newer brands grew 43% last quarter. The pipeline that has to take over in 2029 is only now entering phase 3.

## The technology

Astellas is a large Japanese drugmaker, so its technology is a portfolio rather than one platform. The commercial engine is five Strategic Brands. PADCEV is an antibody-drug conjugate that targets Nectin-4 and [delivers the toxin MMAE into bladder cancer cells](https://newsroom.astellas.com/2026-10-23-european-commission-expands-padcev-enfortumab-vedotin-plus-keytruda-pembrolizumab-approval-to-adults-with-resectable-muscle-invasive-bladder-cancer-regardless-of-cisplatin-eligibility). IZERVAY treats geographic atrophy in the eye. VYLOY targets CLDN18.2 in gastric cancer. VEOZAH is a nonhormonal pill for menopause hot flushes, and XOSPATA treats FLT3-mutated leukemia.

The newer science sits in the pipeline. Setidegrasib is a protein degrader that [marks mutant KRAS G12D for destruction](https://newsroom.astellas.com/2026-09-24-astellas-doses-first-patient-in-phase-3-study-of-setidegrasib-in-previously-treated-kras-g12d-mutated-advanced-non-small-cell-lung-cancer) by the cell's own machinery. ASP2138 is a [subcutaneous bispecific that links CLDN18.2 tumor cells to T cells](https://newsroom.astellas.com/2026-08-25-astellas-doses-first-patient-in-phase-3-study-of-asp2138-in-cldn18-2-postive-and-her2-negative-locally-advanced-unresectable-or-metastatic-gastric-or-gastroesophageal-junction-gej-adenocarcinoma). Astellas has widened its degrader work into what it calls an [induced proximity platform](https://www.astellas.com/content/dam/astellas-com/global/en/confidential-documents/financial-results/1q2026_pre_en.pdf), and it holds rights outside Greater China to ASP546C, a CLDN18.2 antibody-drug conjugate.

## The odds

The marketed brands carry little scientific risk now. PADCEV's newest label rests on a phase 3 trial that [cut the risk of recurrence, progression or death by 47%](https://newsroom.astellas.com/2026-10-23-european-commission-expands-padcev-enfortumab-vedotin-plus-keytruda-pembrolizumab-approval-to-adults-with-resectable-muscle-invasive-bladder-cancer-regardless-of-cisplatin-eligibility) against cisplatin chemotherapy in operable bladder cancer, and it is approved for that use in the U.S. and EU.

The pipeline is a different story. Setidegrasib and ASP2138 are just entering phase 3, where industry data put the chance of reaching an application at [57.8% across all diseases (47.7% in oncology), and the chance of approval from phase 3 at 52.4% (43.9% in oncology)](https://go.bio.org/rs/490-EHZ-999/images/ClinicalDevelopmentSuccessRates2011_2020.pdf). Astellas has missed recently: in March, gilteritinib [failed its overall survival goal](https://newsroom.astellas.com/2026-03-09-astellas-and-hovon-confirm-phase-3-study-did-not-meet-its-primary-endpoint-of-overall-survival-in-patients-with-newly-diagnosed-flt3m-aml) in newly diagnosed FLT3-mutated AML. The company plans [10 or more phase 3 or registrational starts by FY2030](https://newsroom.astellas.com/2026-05-26-astellas-announces-five-year-corporate-strategic-plan-to-deliver-greater-value-faster-for-patients-and-position-itself-as-a-sustainable-growth-company), so on base rates several will fail. The question is whether one or two large wins arrive in time.

## The money

Astellas is profitable and pays a rising dividend. First-quarter revenue [rose 26.7% to 640.9 billion yen](https://www.astellas.com/content/dam/astellas-com/global/en/confidential-documents/financial-results/1q2026_en.pdf) and core operating profit rose 55.6% to 221.4 billion yen. Cash and equivalents were 244.7 billion yen at June 30, against 589.9 billion yen of bonds and borrowings. Operating activities brought in [86.5 billion yen of cash in the quarter](https://www.astellas.com/content/dam/astellas-com/global/en/confidential-documents/financial-results/1q2026_en.pdf), while dividends paid came to 69.9 billion yen and investing activities used 69.2 billion yen. The company guides for [2,220.0 billion yen of FY2026 revenue](https://www.astellas.com/content/dam/astellas-com/global/en/confidential-documents/financial-results/1q2026_en.pdf) and an 80 yen dividend.

The CSP2026 plan sets a [30% core operating margin target by FY2027](https://newsroom.astellas.com/2026-05-26-astellas-announces-five-year-corporate-strategic-plan-to-deliver-greater-value-faster-for-patients-and-position-itself-as-a-sustainable-growth-company) and 200 billion yen of cumulative cost cuts. Astellas' capital policy keeps gross debt to EBITDA at [1.0x to 1.5x](https://www.astellas.com/content/dam/astellas-com/global/en/confidential-documents/financial-results/1q2026_pre_en.pdf) to leave room for a large deal.

## The risks

The largest is XTANDI. It still sold [276.6 billion yen in the June quarter](https://www.astellas.com/content/dam/astellas-com/global/en/confidential-documents/financial-results/1q2026_en.pdf), more than all five Strategic Brands combined, and Astellas' forecast assumes a decline of [about 50 billion yen in FY2026](https://www.astellas.com/content/dam/astellas-com/global/en/confidential-documents/financial-results/4q2025_pre_en.pdf). The second is U.S. pricing. Astellas signed an MFN agreement in August to [lower Medicaid prices and align future launch prices](https://newsroom.astellas.com/2026-09-01-astellas-enters-voluntary-agreement-with-us-government-supporting-affordable-access-to-medicines) with other developed nations, without disclosing all terms. The third is currency: the weak yen [added 60.0 billion yen to first-quarter revenue](https://www.astellas.com/content/dam/astellas-com/global/en/confidential-documents/financial-results/1q2026_en.pdf), and that can reverse.

## Our read

XTANDI sales were still up 18.7% in the June quarter, and the newer brands are growing faster, led by PADCEV as it wins earlier-stage bladder cancer indications. The new plan says pipeline-led growth starts from FY2029. Until then, investors are paid through the dividend and margin expansion. The case beyond 2030 depends on setidegrasib and the CLDN18.2 programs. Those trials will not read out for years; the lung cancer study's registry entry estimates [primary completion in June 2030](https://clinicaltrials.gov/study/NCT07566052). So the near-term story is execution on launches, costs and the 30% margin target, plus whatever late-stage assets business development can add.
