# Arcus bets the company on casdatifan, and now has the cash to do it

The TIGIT program is gone and Gilead has left the board. What remains is a HIF-2 alpha pill with good single-arm data in kidney cancer and more than $900 million to prove it in phase 3.

## The technology

Casdatifan is a once-daily pill that blocks HIF-2 alpha, a protein Arcus describes as [a master switch](https://s202.q4cdn.com/599190870/files/doc_news/Arcus-Presents-New-Data-from-ARC-20-that-Demonstrate-the-Opportunity-for-Casdatifan-to-Become-a-Foundational-Treatment-Across-All-Lin-N7V7C.pdf) that turns on hundreds of genes when oxygen is low. In most clear cell renal cell carcinomas, the most common kidney cancer, genetic changes leave that switch stuck on. One HIF-2 alpha inhibitor is [already approved](https://s202.q4cdn.com/599190870/files/doc_news/Arcus-Presents-New-Data-from-ARC-20-that-Demonstrate-the-Opportunity-for-Casdatifan-to-Become-a-Foundational-Treatment-Across-All-Lin-N7V7C.pdf). Arcus's pitch is that casdatifan blocks the pathway more deeply and for longer.

The data support that pitch so far. In 121 heavily pretreated patients on casdatifan alone, [median overall survival was 35 months](https://www.sec.gov/Archives/edgar/data/1724521/000172452126000047/rcus-20261020.htm) and median progression-free survival 13 months. A paper in Nature linked deeper suppression of blood erythropoietin, a marker of HIF-2 alpha blockade, to [higher response rates and longer progression-free survival](https://s202.q4cdn.com/599190870/files/doc_news/New-Research-Published-in-Nature-Links-Clinical-Activity-with-HIF2a-Biology-in-Advanced-Kidney-Cancer-Patients-Treated-with-Casdatifan-2026.pdf). In second line, casdatifan plus cabozantinib produced a 14.5-month median progression-free survival in 43 patients, and Arcus puts the cabozantinib benchmark at [about 10 months](https://s202.q4cdn.com/599190870/files/doc_news/Arcus-Presents-New-Data-from-ARC-20-that-Demonstrate-the-Opportunity-for-Casdatifan-to-Become-a-Foundational-Treatment-Across-All-Lin-N7V7C.pdf).

The catch is that every one of these numbers comes from single-arm cohorts compared against other trials. Arcus itself warns that its benchmarks are [not head-to-head comparisons](https://s202.q4cdn.com/599190870/files/doc_news/Arcus-Presents-New-Data-from-ARC-20-that-Demonstrate-the-Opportunity-for-Casdatifan-to-Become-a-Foundational-Treatment-Across-All-Lin-N7V7C.pdf).

## The odds

PEAK-1 is the test that matters first. It randomizes immunotherapy-experienced patients to casdatifan plus cabozantinib or cabozantinib alone, the same setting as the 43-patient cohort, and should [finish enrolling by year-end 2026](https://www.sec.gov/Archives/edgar/data/1724521/000172452126000042/a2026q28kexhibit991.htm). Industry-wide, [57.8% of phase 3 programs](https://go.bio.org/rs/490-EHZ-999/images/ClinicalDevelopmentSuccessRates2011_2020.pdf) reach a filing and [52.4% of drugs in phase 3](https://go.bio.org/rs/490-EHZ-999/images/ClinicalDevelopmentSuccessRates2011_2020.pdf) are eventually approved, per BIO, Informa and QLS data for 2011 to 2020. A drug with a validated target and a large phase 1b effect should beat that average, but cross-trial gaps tend to narrow in randomized studies.

The first-line plan carries more risk. PEAK-20 will test casdatifan with ipilimumab and nivolumab, but the triplet cohort that supports it had only [three months of median follow-up](https://www.sec.gov/Archives/edgar/data/1724521/000172452126000047/rcus-20261020.htm) at the cutoff. The doublet with zimberelimab, which showed 69% of patients progression-free at 12 months, is not the regimen going into phase 3.

## The money

Arcus estimated [about $694.0 million](https://www.sec.gov/Archives/edgar/data/1724521/000119312526427829/d130368d8k.htm) in cash and investments at Sept. 30, then priced a $250 million share sale that should bring in [about $234.5 million net](https://www.sec.gov/Archives/edgar/data/1724521/000119312526428916/d117931d424b5.htm). Before the raise, it guided to runway [until at least the second half of 2028](https://www.sec.gov/Archives/edgar/data/1724521/000172452126000042/a2026q28kexhibit991.htm). Research and development spending was $113 million in the second quarter and should fall as TIGIT trials wind down, offset by casdatifan costs.

There are no product sales. Revenue comes from partners, mainly Gilead and Taiho, and full-year 2026 GAAP revenue guidance is [$65 million to $75 million](https://www.sec.gov/Archives/edgar/data/1724521/000172452126000042/a2026q28kexhibit991.htm).

## The risks

Concentration is the main one. Domvanalimab ended after [STAR-221, STAR-121 and PACIFIC-8](https://www.sec.gov/Archives/edgar/data/1724521/000172452126000042/a2026q28kexhibit991.htm) were stopped, so casdatifan carries most of the value, with quemliclustat's PRISM-1 pancreatic cancer readout in the first half of 2027 as the second bet. Gilead held [about 24.7% of the stock](https://www.sec.gov/Archives/edgar/data/1724521/000172452126000044/rcus-20260630.htm) at June 30 and no longer sits on the board. Arcus's prospectuses give a [last reported sale price of $23.75](https://www.sec.gov/Archives/edgar/data/1724521/000119312526427888/d117931d424b5.htm) on Oct. 20 and [$19.10](https://www.sec.gov/Archives/edgar/data/1724521/000119312526428916/d117931d424b5.htm) on Oct. 21, the day it priced a share sale at $17.50. The filings do not say why the shares moved. Anemia and hypoxia are class effects that could limit combinations.

## Our read

Arcus has done what a focused biotech should: dropped a failing program, funded its best asset and lined up partner drugs at low cost. The second-line data look good enough to give PEAK-1 a fair chance. The first-line story is still mostly a plan. Until randomized data arrive, the story rests on enrollment timelines and small cohorts.
