## Fact-check-o-meter

Some gaps

Open about the guidance cut, but the August data release leans on subgroups and post hoc work

Alnylam's quarterly release is direct that it lowered TTR guidance because second-line demand normalized, and its numbers match the 10-Q. The August ESC release is more generous to its data: it features a post hoc zilebesiran subgroup that the 10-Q does not describe as a win, and it presents a prespecified HELIOS-B subgroup as consistent benefit while stating that the trial was not powered for that population. The 10-Q also discloses a U.S. Attorney subpoena that the releases do not mention.

1. Press release vs filing · weight medium Guidance cut explained as demand normalization; competition sits in risk factors **The company said:** "the normalization of growth in second line volume after satisfying pent-up demand from patients waiting for a new therapy" [source](https://investors.alnylam.com/press-release?id=ddc78e15-7847-4066-8fdb-3562023a60f4) **The record says:** The 10-Q does not repeat that explanation. Its risk factors state that "AMVUTTRA is administered through a subcutaneous injection and may not compete favorably with other available options for the treatment of ATTR-CM, including VYNDAQEL/VYNDAMAX, which is marketed by Pfizer" and name an oral competitor from BridgeBio. The release's own revenue detail says U.S. TTR growth of $106 million versus Q1 combined a $129 million demand increase with about $20 million of inventory impact and "a modest reduction in net price." [source](https://www.sec.gov/Archives/edgar/data/1178670/000117867026000062/alny-20260630.htm) **Our view:** The release is candid that guidance fell, and the cause it gives is plausible. The filing keeps competition and pricing as open risks without tying them to the cut, so investors cannot tell from either document how much of the slowdown is one-time pent-up demand versus share.
2. Science · weight medium HELIOS-B tafamidis subgroup: consistent direction, not powered, weaker quality-of-life effect **The company said:** "The treatment effect for vutrisiran on the primary composite endpoint of all-cause mortality and recurrent cardiovascular events through 33-36 months was consistent irrespective of baseline tafamidis use" [source](https://investors.alnylam.com/press-release?id=9c44884f-61bb-4f2e-85d5-09f4659cfc7c) **The record says:** ClinicalTrials.gov lists HELIOS-B as randomized, quadruple masked, 655 participants, with the primary composite assessed up to month 36 in the overall population and, as a second primary outcome, in the vutrisiran monotherapy subgroup. The release itself adds that "HELIOS-B was not powered to establish the benefit of vutrisiran specifically in the population of patients receiving background tafamidis at baseline" (259 of 654 patients) and that the Kansas City Cardiomyopathy Questionnaire improvement showed "an attenuated effect seen among patients receiving tafamidis at baseline." It labels the intrinsic capacity and adverse-event analyses "post hoc". [source](https://clinicaltrials.gov/study/NCT04153149) **Our view:** The release calls the tafamidis analysis prespecified and discloses the limits, which is to its credit, but the headline and quote lead with consistency. For a market where many patients already take tafamidis, the combination question is commercially central and is not answered by this trial.
3. Science · weight medium Zilebesiran: post hoc subgroup featured; the 10-Q describes KARDIA-3 as informing design **The company said:** "In patients who were receiving a background diuretic with an office systolic BP (SBP) ≥140 mmHg at baseline, zilebesiran achieved greater reductions in mean office and 24-hour ambulatory SBP than in the overall study population." [source](https://investors.alnylam.com/press-release?id=9c44884f-61bb-4f2e-85d5-09f4659cfc7c) **The record says:** The release's safety sentence refers to "this post hoc subgroup". ClinicalTrials.gov lists KARDIA-3 as a 375-participant randomized, quadruple masked phase 2 with a single primary outcome, change from baseline at Month 3 in mean seated office systolic blood pressure. The 10-Q says only that KARDIA-3 "met the objective of informing the design, patient population, and dose for a global Phase 3 cardiovascular outcomes trial." [source](https://clinicaltrials.gov/study/NCT06272487) **Our view:** Choosing a diuretic subgroup after seeing results is how phase 3 designs are shaped, but it is not evidence the effect will hold in ZENITH. The 10-Q wording describes the study's role in shaping the phase 3 design and does not report an efficacy result.
4. Legal and disclosure record · weight medium U.S. Attorney subpoena on price reporting; two patent suits; no securities case listed **The record says:** Note 12 of the Q2 2026 10-Q states: "In October 2025, we received a subpoena from the U.S. Attorney's Office for the District of Massachusetts seeking documents pertaining to our government price reporting for AMVUTTRA, ONPATTRO, OXLUMO and GIVLAARI", and that Alnylam has produced records and is "continuing to engage with the U.S. Attorney's Office." It also lists a patent suit by the Board of Regents of the University of Texas System over ONPATTRO, filed December 12, 2024 and now in the U.S. District Court for the District of Massachusetts with a claim construction hearing held in July 2026, and a March 13, 2026 suit by Alnylam and Novartis against Cipla in the District of Delaware over a generic Leqvio application. The 10-Q says "There were no material loss contingencies accrued as of June 30, 2026 or December 31, 2025." No securities class action is listed in Note 12. [source](https://www.sec.gov/Archives/edgar/data/1178670/000117867026000062/alny-20260630.htm) **Our view:** A subpoena is an information request, not a finding, and the filing reports no charges. It covers all four marketed products, though, and the Q2 release does not mention it.
